Job summary

We are looking for a Practice Nurse to join our Team.

Job Summary:

The Practice Nurse will be able to assess the nursing needs of patients. They will be expected to provide appropriate care/treatment in conjunction with the GP, or independently, as appropriate and according to Practice policy, protocol etc. The Practice Nurse will advise patients in general health care and minor ailments with referral to General Practitioners when necessary.

Main duties of the job

Job Requirements:

  • Identify, and manage as appropriate, treatment plans for patients both at risk of developing a long-term condition and diagnosed chronic disease
  • Undertake cervical cytology screening
  • Dressing and wound management, including compression bandaging/hosiery and Doppler/leg Ulcer care.
  • Respiratory Care Asthma & COPD
  • Diabetes Care
  • Suture and clip removal.
  • Management of skin disorders.
  • Recognition of mild childhood illnesses.
  • Testing pathology samples e.g. blood, urine.
  • Blood pressure and weight management.
  • Aid/chaperone to GP during clinical procedures, e.g. minor operations etc.
  • Specimen collection e.g. msu, pregnancy tests, occult blood, faeces for O&S, sputum, etc.
  • General observations e.g. BP, pulse and temperature.
  • ECG
  • Use and care of nebulisers.
  • The Practice Nurse will be responsible for upkeep and maintenance of medical equipment.
  • Ensure the maintenance of stock levels in the Treatment Room relating to patient care.

Vaccinations/Injections

  • The Nurse will administer childhood immunisations as per the child immunisation programme and have signed relevant Patient Group Directives.
  • Travel vaccination schedules and travel advice.
  • Flu vaccination clinics/any other vaccination clinics as deemed appropriate.
  • Any other routine injection e.g. B12.

About us

As part of a multi-disciplinary team we aim to treat and manage the needs of our patient demographic to improve the health of the local community. Working in our modern purpose built medical centre with a diverse practice population of approx. 19,000, you will never be bored.

Our Nursing team consists of 1 phlebotomist, 5 HCAs, 4 Practice Nurses and 4 Nurse Practitioners managed by a Clinical Nurse Manager and Nursing Team Manager. We are looking for a new Practice Nurse to join our Nursing team for 28.5 hours a week.

As a practice we provide teaching for medical students, participation in research studies and participation in service transformation pilots.

Job Summary

To provide care that meets the practice populations needs. To provide health promotion and contribute towards prevention of potentially negative health outcomes and/or hospital admissions.

To participate as required in practice-based research projects and audit.

Job Responsibilities

The post holder will:

  • Ensure clinical practice is safe and effective and that it remains within the boundaries of competence, acknowledging personal limitations.
  • Maintain continual professional education required for remaining competent within your role.
  • Attendance at training deemed necessary by the practice including annual concordance with statutory and mandatory training.
  • Actively participate in the supervision and appraisal including maintaining a personal development plan.
  • Advise patients on general health care, arranging investigations or referring to other members of the primary health care team as appropriate.
  • Be able to offer health promotion to patients including chronic disease management, nutritional/weight management advice and smoking cessation.
  • Provide assessment, diagnosis and screening advice for new conditions and chronic disease.
  • Provide routine nursing team care such as administering medications, wound care and annual condition reviews.
  • Adhere to current practice PSDs/PGDs as appropriate within the role.
  • Participate in the induction/training of GP registrars, students and practice peers.
  • Undertake home visits if required.
  • Appropriately document all patient interaction in a timely manner insuring it meets legal, professional and practice coding requirements.
  • Adhere to practice processes for reporting of patient concerns, near miss events, errors and safeguarding.
  • Attend meetings such as clinical governance, clinical education meetings and nursing team meetings as required.
