Leeds Student Medical Practice and The Light Surgery Primary Care Network (LSMP and TLS PCN) are looking to recruit a PCN Social Prescriber. Our PCN is extremely diverse with patients mainly being university students or young professionals. We want the most resilient people to join our ever-expanding team.

The Social Prescriber will enhance the patient experience within the PCN by providing a mix of reactive and proactive communication and effective care navigation. This role is crucial in connecting patients with various services, ensuring they receive timely and comprehensive support. The Social Prescriber will address immediate needs, proactively educate patients about available resources, assist in navigating the healthcare system, and foster strong relationships between patients, providers, and community services. By bridging gaps in healthcare delivery, the Social Prescriber will ensure a positive and inclusive experience for all patients within the PCN.

This role is initially a 12-month fixed term contract with potential to extend dependant on the success of the role. We are open to both full-time and part-time hours for this role.

Main duties of the job

The successful applicant will be required to conduct both face to face and telephone appointments for patients referred into the Social Prescribing service. You will be responsible for a caseload of patients across both practices in the network, conducting initial assessment, as well as follow-up appointments as part of an extended intervention.You will be required to work on-site at both Leeds Student medical Practice and The Light Surgery.

The successful applicant will be required to work on their own initiative as well as being a good team player. You will need to be a good listener, empathetic, and have an excellent understanding of patient needs. You will also have outstanding interpersonal skills and be able to work with patients to identify wider health, wellbeing, and social interventions.

PCN Overview:

LSMP and The Light PCN is 1 of 19 Primary Care Networks across Leeds, serving over 63,000 patients through our two practices: Leeds Student Medical Practice and The Light Surgery.

Our multidisciplinary team is composed of highly trained professionals, including clinical pharmacists, pharmacy technicians, occupational therapists, care coordinators, mental health advisors, health and wellbeing coaches, social prescribers, mental health nurse specialists, and community wellbeing connectors. We aim to enhance general practice support by offering a tailored range of services to meet the unique needs of our patient population and improve access to appointments.

We are committed to quality improvement and addressing health inequalities through various projects designed to enhance patient care. By collaborating closely with our Local Care Partnership (LCP) and third-sector organisations, including universities, we strive to meet the needs of our growing community.
