PCN Health and Wellbeing Coach - 37.5 hours per week with satellite working at The Edge on the University of Leeds campus

Leeds Student Medical Practice and The Light Surgery Primary Care Network (PCN), in partnership with the University of Leeds, are looking to recruit a PCN Health and Wellbeing Coach. This is a newly created exciting role to join our ever expanding PCN team. This role will be seconded to the University of Leeds and will be predominantly based at The Edge on the University of Leeds campus.

Our PCN is extremely diverse with patients being mainly students or young professionals. You will join the Physical Activity and Wellbeing team at the University of Leeds that works to improve and support the mental and physical health of the staff and student community. We want the most resilient people to join us- no two days will be the same.

The role is extremely varied and will encompass a wide range of health and wellbeing interventions for PCN patients and the University of Leeds community to make positive lifestyle changes and engage in proactive wellbeing conversations. You will need to be creative and take a holistic approach that enables patients to access wider community support via The Edge gym and self-directed health education.

Main duties of the job

To be successful in this role you will be required to work on your initiative as well as a being a team player which is essential. You will need to be a good listener and empathetic towards patients health and wellbeing needs. You will also have outstanding time management and interpersonal skills and work with patients to identify the best possible health and wellbeing outcomes. You will have experience in a physical activity or health setting and designing individualised support plans.

About us

The PCN has approximately 57,000 patients between both practices and our PCN population is extremely diverse. The Health and Wellbeing coach will be an integral part the PCN team working in collaboration with the University of Leeds. The post holder will work in partnership with the clinical and non-clinical colleagues and wider PCN team to ensure the role delivers the best possible outcomes for patients.

Seconded to the University of Leeds, you will be a key member of the Physical Activity and Wellbeing team, working to improve and support the mental and physical health and wellbeing of the university community in addition to the local community.

The Health and Wellbeing Coach will be responsible for assisting patients in their ability to self-manage motivation levels and improve lifestyle choices. The post holder will be in an expert in behaviour changes and be focused on improving health related outcomes by working with patients to set personalised goals.

This PCN is in a process of moving from a lead practice model to an incorporated company model and as such the post holder will be subject to TUPE provisions if commencing employment prior to incorporation.

Job description

Job responsibilities

1. Delivery of a high-quality patient facing service across the PCN and to support the diverse patient population

2. To work as part of a multi-disciplinary team both within the University of Leeds and PCN in a patient facing role to assess and treat patients using expert knowledge

3. Providing one-to-one and group health coaching support for individuals with health conditions, with the aim of improving peoples knowledge, skills, and confidence in managing their condition/s, empowering people to manage their own health and improve their health outcomes and support them in making changes in their health-related behaviour.

4. To be responsible for the assessment of patients with production and completion of individual care plans

5. To coach and motivate patients through multiple sessions to identify their needs, set goals, and support them to implement their personalised health and care plan.

6. To provide personalised support to individuals to ensure that they are active participants in their own healthcare and to empower them to take more control in managing their own health and wellbeing, to live independently and to improve health outcomes through the following:

7. Providing interventions such as self-management, education and peer support

8. Supporting people to establish and attain and build on goals set by the patient based on what is important to them

9. Supporting wider wellbeing work within the Physical Activity and Wellbeing team at the University of Leeds. This includes physical activity and wellbeing classes and educational workshops.

10. Working with social prescribers and other PCN colleagues to connect them to community based activities which support their health and wellbeing

11. To be aware of limitations and as required be responsible for the referral of patients who require the intervention of other healthcare professionals.

12. To work with patients and to understand their level of knowledge, skills and confidence when engaging with their health and wellbeing.

13. To identify risk factors and categorise patients into appropriate risk handling.

14. To be responsible for the day to day planning of own caseload and workload.

15. Working flexibly, adapting to the needs of the service and patient group while maintaining the integrity of the role;

16. Participating in regular health coaching supervision and continual learning;

17. Working closely within the multidisciplinary team and with GP practices within the PCN to ensure that the relevant SNOMED codes to record activity are inputted into clinical systems (as outlined in the Network Contract DES), adhering to data protection legislation and data sharing agreements.

18. Working as part of a mental health focused multidisciplinary multi-agency team to promote health coaching and to be ambassadors for Personalised Care and Supported Self-Management;

19. Ensuring that GPs, practice nurses, practice pharmacists, social prescribers and other members of the primary care team understand the Health and Wellbeing Coach role, how to refer to them, and which patients may particularly benefit from health coaching;

20. To work with the PCN team to review and develop standard operating procedures relating to the role.

Working Relationships:

To work as a flexible member of the PCN providing support to other team members when necessary.

  • Develop and maintain effective wroking relationships within the University of Leeds.

The post holder will provide line management function to other PCN staff, with support from the HR manager.

Encourage supporting staff to collaborate through sharing information and intelligence across different activities.

Maintain a well-established and collaborative relationship with Leeds GP Confederation and West Yorkshire ICB.
