Job summary

The Riverside Practice are looking to recruit a Full Time Practice Nurse (or a job share of two nurses) to work within our GP Practice. We are ideally looking for an experienced Practice Nurse with Chronic Disease Management, however we will consider someone looking to transition into Primary Care and carry out the training required to become a Practice Nurse.

This is a full time role working 37.5 hours per week (over 4 or 5 days). This position will complement our existing team delivering high quality patient orientated care services to a diverse patient population.

The Riverside Practice is based in March, Cambridgeshire and has 2 GP Partners, 1 salaried GP, 2 Advanced/Nurse Practitioners, 3 Practice Nurses, 3 HCAs and a phlebotomist that look after our patients

Main duties of the job

  • Health promotion and chronic disease management
  • Provide a full range of nursing services to the patients
  • Undertake diagnostic, health screening, health surveillance and therapeutic interventions
  • Contribute to clinical audits including infection control
  • Treatment room duties such as phlebotomy/ wound care/smears/Child/Adult immunisations
  • Carry out mass vaccinations such as Seasonal Flu/Covid
  • Ability to communicate sensitive information with empathy

Associated administrative duties such as protocols, pathology, tasks, clinical admin.

Job description

Job responsibilities

Working in accordance with the NMC Code of Professional Conduct, the post-holder will participate in the development of the practice nursing team in the provision of assessment, treatment, screening, health education services and advice to patients.

The post-holder will in conjunction with the Nurse Manager develop practice agreed protocols and guidelines to provide nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.

The post-holder will have the required level of training and competence and will work within patient group directions where these are available. A nurse with prescribing qualifications will use them appropriately.

Clinical Responsibilities:

Providing assessment, screening and treatment services and health education advice

  • Health promotion and chronic disease management.
  • Provide a full range of nursing services to the patients.
  • Undertake diagnostic, health screening, health surveillance and therapeutic interventions.
  • Contribute to clinical audit.
  • Treatment room duties such as wound care.
  • Carry out mass vaccinations such as Seasonal Flu.
  • Implement and participate in vaccination and immunisation programmes for both adults and children
  • Assist senior practitioners in providing minor surgery sessions.
  • Act as a chaperone to colleagues and senior practitioners as needed.
  • Communicate sensitive information with empathy
  • Any other reasonable clinical duties as directed by the Department Supervisor, The Practice Manager or Partner.
Pathological specimens and investigatory procedures

Following appropriate training, undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc and ensure the cleanliness of any equipment so used.

If appropriate training been undertaken, perform any investigatory procedures requested by the GPs.

Administrative and professional responsibilities

Participate in the administrative and professional responsibilities of the Practice team.

Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate.

Ensure accurate completion of all necessary documentation associated with patient health care.

Ensure collection and maintenance of statistical information required for regular and ad hoc reports.

Attend and participate in Practice meetings as required.

Assist in the formulation of Practice philosophy, strategy and policy and develop appropriate protocols.

  • Any other reasonable administrative duties as directed by the Department Supervisor, The Practice Manager or Partner.

Development

  • Work effectively and efficiently to provide a high quality service to patients.
  • Analyse, interpret and report data on health and well-being, related needs and service use.
  • Attend appraisals with Nurse Manager and Practice Manager appropriate.
  • Innovate in the realms of better working or improved clinical practice. Encourage other team members to share their ideas.
  • Delegate tasks from the above as appropriate and safe to the rest of the nursing team.
  • Identify and present business/clinical cases for nurse training and development to the Nurse Manager.
  • Mentor, develop and support the nursing team as required.
  • Be aware and stay informed of future challenges and changes within primary care nursing. Keep the Nurse Manager informed of potential concerns and future requirements. Work with the Nurse Manager to manage these requirements.
  • Assist the Nurse Manager and Practice Manager to ensure that QOF and the enhanced services are delivered as appropriate.
  • Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council (NMC)
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
  • Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.

Management of Risk

  • Critically review and improve clinical governance and risk management systems.
  • Make recommendations as to good practice and procedure.
  • Monitor policies, standards and guidelines across the practice nursing team (clinical and, where appropriate, non-clinical).
  • Participate in and manage clinical audit.

Research projects

Co-operate and participate as required in any research projects within the Practice.

Student/staff education and training

Participate in the education and training of students of all disciplines and the induction of all members of Practice staff with special responsibility for nursing staff.

Liaison
  • Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the Practice, with appropriate regard to confidentiality.

  • In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately

Practice Protocols:

  • The practice has Protocols in place for Clinical and Administrative tasks, staff are required to adhere to all Practice Protocols, it is essential that all staff keep up to date with protocols and advise of any changes needed. All protocols are stored on the Practice Intranet.

The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety Policy, the practice Health & Safety Manual, and the practice Infection Control policy and published procedures. This will include:

  • Using personal security systems within the workplace according to Practice guidelines
  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
  • Making effective use of training to update knowledge and skills
