Bradford Student Health Service are looking to recruit a Practice Nurse (a minimum of 2 years primary care experience essential)

Our new Practice Nurse can expect to work in an encouraging, supportive, forward thinking, experienced Practice environment that is continually working to improve the quality of healthcare for our patient population.

We are keen to recruit an experienced Practice Nurse to join our friendly, professional, caring team who are passionate about providing safe and effective care to our patient population.

We are able to offer 24 hours per week, competitive salary based on experience. Hours to be worked Monday, Tuesday, Thursday and Friday (6 hours per day - start and finish times tbc).

Our Practice Nurse role would be expected to undertake a range of nursing assessments and provide appropriate care or treatment in conjunction with our clinical team and Practice Policies.

You will be required to deliver a high level of quality care based on the needs of the patient population, which would include treatment, management plans, screening, preventative care and patient education. In addition to this, you would also be expected to support the Practice in meeting its contractual obligations and delivery of enhanced service specifications.

About us

Bradford Student Health Service is a busy GP practice in Bradford with approximately 9,500 registered patients. We are a University practice situated in close proximity to the University of Bradford.

Our clinical team consists of two GP partners , locum GP's , a trainee Advanced Care Practitioner, a Health Care Assistant, three Clinical Pharmacists and a Pharmacy Technician. The clinical team are support by a Practice Manager, Operations Manager, Administrators and a Medical Secretary. Our reception team work as Care Navigators providing enhanced signposting and ensuring patients have access to the appropriate clinicians or services available to them.

We are a member of PCN 6.

The practice is a consistently high performer in relation to its contractual obligations including QOF and Quality Contract services.

Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

To act as or support the Infection Prevention Control (IPC) lead and to work to the IPC Policy providing or supporting the implementation of audits, training, support and evidence of compliance to the practice manager

To produce or support the production of the IPC annual statement in conjunction with the practice manager

To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

To develop, implement and embed health promotion and wellbeing programmes

To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health

As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

As required, to diagnose and manage acute and chronic conditions

To review patient medications to enhance compliance

To co-ordinate the provision of travel medicine services

To request pathology services and process pathology results as required

To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

To maintain accurate clinical records in conjunction with extant legislation

To ensure read codes are used effectively by all members of the nursing team

To ensure the appropriate maintenance and use of chronic disease registers

To develop, implement, embed and participate in well woman clinics

To develop, implement, embed and participate in well man clinics

To ensure compliance with the practice chaperone policy

To prioritise health issues and intervene appropriately

To support the team in dealing with clinical emergencies
