Job summary
We are looking to recruit a registered Nurse with experience in delivering the management of diabetes.
Selsey Medical Practice are part of
Chichester Association of Medical Practice Primary Care Network.
We are committed to improving the care provided to patients, reducing
health inequalities and raising the quality and standards of GP practice. This
role will part of the developing model of care which addresses our ambition to
deliver person-centred coordinated care and support implementation of the NHS
long-term plan.

Main duties of the job
Practice nurses are part of our multi disciplinary team delivering chronic disease
management clinics.
The successful candidate will:
- Provide compassionate, safe and
effective care and support to patients.
- Prioritise and manage their own workload,
recognising where elements of care can safely be delegated to other colleagues,
carers and family members.
- Provide and receive sensitive information
concerning patients medical conditions

About us
Selsey Medical Practice is part of the Chichester Alliance of Medical Practices (ChAMP) Primary Care Network which consists of 8 GP surgeries covering Chichester, Selsey, Wittering, Tangmere and Southbourne and provides care to over 92,000 patients.
We have 12,500 patients registered at our practice, we have 11 GP's who are supported by a team of nurses and other healthcare professionals and supporting roles.
We are a training practice, with 2 GP trainers.
