Job summary

The Emmett Carr GP Partnership is looking for a new Advanced Nurse Practitioner/Advanced Clinical Practitioner to join an experienced team of doctors, nurses and HCAs and a wider clinical and administration team. The ideal candidate would be dynamic, committed, and capable of maintaining positive relationships within the team. We are looking for someone to work Part time, over two days .

Main duties of the job

You will have clinics of both face to face and telephone contacts, which will include a mix of pre-booked and on the day appointments covering the full range of clinical experience

Your role would be to deliver effective healthcare to our patients, including initial history taking, clinical assessment, diagnosis, treatment, prescribing and evaluation of care.

We have an exciting opportunity to welcome an Advanced Clinical Practitioner or Advanced Nurse Practitioner to our team.

We are a long established, caring and supportive Medical Practice based on the Sheffield Derbyshire border working across two sites which are 2 miles apart offering care to 4900 patients. We have 2 GP partners and 2 salaried GP's. A community Matron, Care Coordinator, Clinical Pharmacist, 2 Nurses, 2 Health Care Assistants, Mental Health Support Workers, Paramedic, Social Prescriber's. We have a well established Admin and Reception team with advanced Care navigation and reception based triage which help to ensure that patients are booked with the correct clinician.

We are part of the North Hardwick and Bolsover Primary Care Network.

Work autonomously and be accountable for his/her own professional actions.

Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services.

Discuss with GP and, where appropriate, make referrals

Prescribe medication as appropriate.

Maintain accurate record keeping in System One.

Request and interpret tests appropriate to patients need.

Reduce inappropriate demand for emergency services by referring patients to appropriate health and social care agencies.

Deliver evidence based clinical practice by utilising applied knowledge of clinical audit, critical appraisal of research, research methodology, ethical review and research governance.

Make appropriate use of clinical decision support software and guidance documents.

Work as an effective member of a multi-professional team. Maintain an up-to-date knowledge of clinical practices, ensuring that these are reflected in compliance with policies and procedures.

Communicate effectively with staff and managers on a regular basis, both individually and at team meetings.

Recognise peoples needs for alternative methods of communication and respond accordingly when communicating with patients and carers.

Assist the GPs, Managers and Partners by promoting and establishing a healthy and safe working environment for all personnel. Provide clinical advice and feedback on matters of health and safety, risk assessments, accident reporting and investigations.
