Filey and Scarborough Healthier Communities Network
are looking to recruit a highly motivated and enthusiastic Clinical Pharmacist
to join our expanding and growing multidisciplinary healthcare network team.
The post holder will work with a diverse range of
people from different cultural and social backgrounds and will ensure that our
PCN practices integrate with community and hospital pharmacy to help utilise skill
mix, improve patient outcomes, ensure better access to healthcare and help manage
workload. The role is pivotal to improving the quality of care and operational efficiencies,
and you will work in partnership to provide clinical reviews and medicines
management in line with current guidance across a wide range of therapeutic areas.
The ability to work confidently and effectively in a varied and sometimes
challenging environment is essential.
Applications welcome from pharmacists who have
experience of working in any sector of health and social care, that includes
having patient-facing experience and direct contact with people, families or
carers. Experience in general practice
and qualification as an independent prescriber is desirable, but full training will
be offered as part of the development plan. As a PCN pharmacist you will receive
fully accredited training from CPPE.
The successful candidate will have excellent
interpersonal and communication skills, and be organised, patient and empathetic.
Main duties of the job
Responsible for supporting the PCN in implementing
effective medicine management across the patient demographic with a primary
focus on the Enhanced Health in Care Home Framework, complex and problematic
polypharmacy and medication safety. The role will be key in identifying areas
for improvement, as well as initiating and managing change. To do this you will
combine patient facing, analytical and administrative elements of work. will
provide primary support to general practice staff with regards to prescription
and medication queries. They will help support the repeat prescription system,
deal with acute prescription requests, and medicines reconciliation on transfer
of care and systems for safer prescribing, providing expertise in clinical
medicines advice while addressing both public and social care needs of patient
in the GP practice

and Transformation Manager and a Business and Operations Officer.

Primary Responsibilities
The following are the core responsibilities of the Clinical Pharmacist. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- Carry out telephone and face-to-face consultations with patients and/or carers with medication queries ensuring accurate notes of consultations are recorded
- To provide medication review services via face to face or remotely (via telephone or video consult) with patients/carers.
- Work with nursing staff and other community/PCN pharmacists for medication reviews for patients in local nursing/residential homes or for housebound patients
- To provide advice and support for patients with self-limiting conditions
- Encourage cost-effective prescribing across the PCN
- Review all safety alerts ensuring the required actions are taken
- To reconcile medicines following patients discharge from hospital or clinic appointments
- To discuss with patients, if appropriate, any changes in treatment from discharge or clinic letters
- Review medications for newly registered patients
- Work with practice to ensure a robust repeat prescribing system
- To provide subject matter expertise on medication monitoring, implementing and embedding a system
- Carry out targeted medication reviews for high-risk patients
- To re-authorise medication for repeat prescribing within scope of practice
- Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required
- Review and implement safe prescribing systems for vulnerable patients, improving compliance
- Advise clinicians on any new guidance pertaining to medications
- Actively signpost patients to the correct healthcare professional
- Assist with Quality Outcome Framework targets
- Monitor practice prescribing against the local health economys RAG list and make recommendations to GPs for medicines that should be prescribed by hospital doctors (red drugs) or subject to shared care (amber drugs).
In addition to the primary responsibilities, the clinical pharmacist may be requested to:
- Partake in audit as directed by the audit lead
- Provide assistance with dispensary matters
- Provide education on medicines for the clinical team
- Review the latest guidance ensuring the practice conforms to NICE, CQC etc.
- Act as the practice point of contact for all medicines related matters, establishing positive working relationships
