Job summary

Palace Road Surgery is based in Streatham Hill near the South Circular. We are looking for an experienced ACP to join our team.

Breakdown of the practice

Friendly, supportive, innovative practice, strong clinical support team onsite including GPs, Practice Nurses, Pharmacists, Physios, HCA, Social Prescriber, Health Coach, Care Coordinator, GP Assistant, Substance Misuse Counsellor, CVD Clinic and SLAM services on the first floor.

We are a paper-light practice, we have a strong team ethos in a friendly supportive working environment. Practice/Finance Manager, Operations Manager on site all times. Regular weekly clinical meetings.

Main duties of the job

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan. clinical care accordingly.

The post holder is an experienced ACP who, acting within their professional boundaries will provide care for the presenting patient from triage, initial history taking, clinical assessment, diagnosis, treatment and evaluation of care and triage.

They will demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills, for patients within the general practice. The post holder will demonstrate critical thinking in the clinical decision making process.

They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing clinical leadership as required. In order to work at this level advanced practice must be met.

To be considered for this exciting role you must have: experience in a Primary Care setting, hold a degree in a health-related discipline, be currently registered with one of the statutory regulators of health and care professions, be employed as a health care professional.

We are a warm and friendly practice, which offers really interesting and varied patient caseloads, and a fantastic work-life balance. Youll be joining a well-established primary care team working with five other Streatham PCN practices. We have a mixed patient population with many generations of patients who have stayed registered with our practice for many years. We see ourselves as a traditional family GP Surgery and have established a good rapport with our patients. We are a training practice with a list size of just under 8000 patients.

Job description

Job responsibilities

The following are the core responsibilities of the ACP. There may be on occasion a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels. You will work within your own scope of clinical practice and:

a. Provide clinical leadership and supervision colleagues.

b. Provide planned or unplanned care to patients as required, in accordance with clinically based evidence, NICE guidance, the Enhanced Health in Care Homes Framework and the Direct Enhanced.

c. Triage, examine, diagnose offer treatment, or liaise on patient care with the most appropriate member of the clinical team as required.

d. Maintain accurate clinical records in line with legislation, ensuring read codes are used correctly.

e. Ensure continuity of care, arranging follow-up consultations or reviews as necessary.

f. Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes.

g. Recognise, assess and refer patients presenting with mental health needs where appropriate.

h. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice).

i. Acting in accordance with local and National safeguarding policies and procedures and having an awareness of vulnerable patients in our community.

j. Liaise with external services/agencies to ensure that the patient is supported appropriately (vulnerable patients, etc.) .

k. Ensure that they adhere to the relevant patient group directives and local clinical pathways at all times.

l. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.

m. Deliver opportunistic health promotion where appropriate.
