Job summary
We are looking for a compassionate and skilled Practice Nurse to play a vital role in delivering high-quality, evidence-based care to our diverse patient population. You will take the lead in supporting patients with long-term conditions, including elderly and housebound patients, making a real difference to their day-to-day health and wellbeing.
Working as a valued member of our multidisciplinary team, you will deliver a wide range of nursing services including chronic disease management, infection prevention and control, health promotion, and well woman and well man clinics. You will also have the opportunity to contribute to the development and review of clinical policies and procedures, helping shape best practice within the surgery.

Main duties of the job
The Practice Nurse will deliver high-quality, evidence-based nursing care to support patients across all stages of health and illness. Key duties include leading and supporting infection prevention and control activities, developing and delivering care management programmes for frail, elderly, and housebound patients, and managing chronic and long-term conditions through assessment, treatment planning, and ongoing review.
The role involves health promotion and wellbeing initiatives, vaccination programmes, well woman and well man clinics, travel health services, and the diagnosis and management of acute and chronic conditions within scope of practice. The post holder will request and interpret pathology results, maintain accurate clinical records and disease registers, and ensure effective use of clinical coding.
Working closely with the multidisciplinary team, the Practice Nurse will support safeguarding, mental health assessment and referral, clinical emergencies, and liaison with external agencies to ensure holistic care for vulnerable patients. The role also includes contributing to policy development, audits, staff support, and delivering opportunistic health promotion to improve patient outcomes.

Crossley Street Surgery is a progressive, four-partner GP practice serving a semi-rural population of around 12,000 patients from modern, purpose-built premises in Wetherby.
We are a teaching and training practice, hosting GP registrars and FY2 doctors throughout the year.
