Job summary

We are looking for an experienced Practice Nurse to join our already experience nursing team. Ideally we are looking for 24 hours over 3 days, and can be flexible for the right person.

Experience in Chronic Disease Management in particular Diabetes/COPD/Asthma is essential, holding a Diploma for some or all of the disease areas is required.

The successful candidate will be responsible for providing comprehensive nursing care to patients including immunisations, cytology, wound care and other clinical duties. They should have excellent communication skills. A good knowledge of QOF is desirable. We would also welcome Practice Nurses with Non Medical Prescribing Certificate and Minor Ailments Certificate to apply.

Main duties of the job

You will provide a comprehensive nursing service to the practice population to meet healthcare needs. You will undertake a variety of activities mainly including managing long term condition reviews for Diabetes, COPD, Asthma, CKD, HF, CHD. Also other nursing duties such as cytology, immunisations, and dressings.

We are a very supportive and friendly practices, multiple staff members have been with the practice over 30 years. We have a total compliment of over 30 staff over 2 sites including GP's, Nurse Practitioner's, First Contact Practitioners, Practice Nurses, Nurse Associate, HCA, Phlebotomist and Pharmacist.

We work with and are supported by numerous ARRS staff.

We are are always happy to help each other and always there to celebrate each others achievements.

Venepuncture:Carry out venepuncture procedures following local protocols to facilitate diagnostic testing and patient care.
