Job summary
We are
seeking an experienced Clinical Pharmacist with prescribing qualifications to
join our dynamic primary care team. This is an exciting opportunity to work
directly with patients and healthcare professionals in a busy GP surgery,
providing comprehensive pharmaceutical care and contributing to improved patient
outcomes.
The
successful candidate will work collaboratively with GPs, nurses, and other
healthcare professionals to deliver high-quality clinical pharmacy services,
including medication reviews, chronic disease management, and prescribing
within their scope of practice.
Main duties of the job
- Conduct comprehensive
medication reviews and optimisation for patients with complex medication
regimens
- Provide independent
prescribing services within scope of practice and competency
- Manage patients with
long-term conditions including diabetes, hypertension, COPD, and
cardiovascular disease
- Deliver medication adherence
support and patient education
- Conduct clinical
consultations and follow-up appointments
- Participate in
multidisciplinary team meetings and case discussions
- Support practice audit
activities and quality improvement initiatives
- Provide medicines
information and advice to patients and healthcare professionals
- Maintain accurate clinical
records and documentation
- Participate in clinical
governance activities and incident reporting
About us
Southbourne Surgery is a located nearby to Chichester and Emsworth. There is easy access to the South Downs and to Chichester Harbour. We are a hardworking team who also enjoy socialising.
The current clinical team comprises nine GPs, five nurses, three HCAs, a clinical pharmacist, pharmacy technician, two paramedics, a Physician Assistant. We also have a number of care coordinators, administrative staff and patient services advisers who provide back office support.
We welcome visits to the surgery to meet the team.
