Job summary

We are looking to grow our team of Practice Nurses, we are a busy, friendly practice. The Practice Nurse role will be to see patients for their annual chronic disease check, diabetes, asthma, hypertension etc. carry out cervical screening, give injections e.g. baby immunisations and holiday vaccinations. To carry out first dressings when necessary, and help run annual flu & Covid clinics. To be available to help our HCA's when needed. Deal with patients face to face or by telephone, and to carry out home visits for our housebound patients. You will also be required to work an improved access shift which is 6.30-8.00 pm.

Main duties of the job

Ideally you will have experience in General Practice although training will be given to successful candidate if needed. You must be flexible, reliable and willing to cover some sickness or holiday. The ideal hours for this role will be 4.00pm to 8.00 pm 3 days per week, but this is negoitable.

About us

We are busy, friendly hard working practice, our team comprises of 3 GP Partners, 2 salaried GPs, 3 Nurse Practitioners/ANPs, 5 practice nurses, 5 HCAs, 2 Pharmacists, 3 Pharmacy technicians. We have a large admin/reception team.

We work over 2 sites and have approximately 15500 patients.

We are part of the Lowestoft PCN and have PCN staff working across all Lowestoft practices supporting us all.

PRACTICE NURSE

RESPONSIBLE TO

Clinically to the Senior Nurse

Administratively to the Practice Manager

MAIN PURPOSE

To work as a team member with the G.P. colleagues and deliver optimum patient care. Maintaining responsibility and accountability for all care given by the Nurse.

Responsibility to ensure that all nursing care delivered to the Practice population is of the highest standard, is effective and the patient needs are met and continually assessed.

Maintaining responsibility for the nursing care needs of patients identified by the Nurse and where care has been transferred to her by the G.P.

Responsibility for identifying care needs within the Practice population (i.e. screening) and consequently organising work to meet the needs identified.

Direct involvement in Protocol and policy development and overall management of patient care, providing the nursing input to any decision-making processes affecting patient care.

Liaison with other members of the Primary Health Care team in order to provide continuity and effective desired overall care of Practice patients.

Management of the nursing and medical care of those patients with chronic Disease with the implementation of agreed Protocols. Maintaining a Disease Register and recall systems for these patients. Continual assessment and review of these patients and initiating treatment changes, self-management guidelines as appropriate.

Ability to demonstrate the rationale behind all Nursing care implemented, to continually assess, evaluate and alter the treatment given as necessary.

Responsibility of Practice Nurse to work independently as an autonomous accountable Practitioner.

Reflecting on workload, current practice, ensuring that patient safety is never compromised.

OTHER MAJOR DUTIES AND RESPONSIBILITIES

  1. Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs.

  2. Cytology

  3. Wound care

  4. Removal of sutures

  5. Travel advice and immunisations

  6. ECGs

  7. Hypertension management

  8. Ear syringing

  9. Routine immunisations/childhood immunisations

  10. Requesting pathology tests, for example urine culture, swabs

  11. Following agreed clinical protocols with referral to a senior nurse or GPs as appropriate

  12. Pill checks/HRT

  13. Minor illness

  14. Health promotion

  15. Participation in administrative systems in the practice

  16. At all times there will be a need to maintain accurate records

  17. A duty to advise senior nurse of potential problems or errors within the range of assigned tasks

  18. Attend and participate in any practice meetings when required

  19. On occasion there may be a requirement to undertake home visits

  20. Involvement and participation in practice research activity

  21. Any other delegated duties appropriate to the post
