We are looking to employ an
Enhanced Practice Nurse (to be employed through the ARRS Scheme) to work out of
The Bellingham Practice in rural Northumberland, for 16 hours (2 days) a week.
You will work as part of a
multidisciplinary team to deliver enhanced clinical care. As an autonomous
practitioner, you will be responsible for the care delivered, and will need to
demonstrate critical thinking and clinical decision-making skills. You will
provide care for patients from initial history taking, clinical assessment,
diagnosis and treatment. As an Enhanced Practice Nurse, you will also
contribute to and attend education and training to other nurses,
multidisciplinary team members and students. You will help us to transform and
modernise pathways of care, enabling the safe and effective sharing of skills
across professional boundaries.
Main duties of the job
Works as part of a MDT within
the PCN.
Consults with patients, their
family, and the multi-professional team to undertake assessments of patient
need and devise and evaluate complex care plans.
Evaluates and analyses clinical
problems using their clinical knowledge, seeking out and applying relevant
evidence, enhanced techniques, interventions, and equipment to make clinical
decisions.
Delivers enhanced clinical care
in the context of continual change, challenging environments, different models
of care delivery, innovation and rapidly evolving
technologies using analysis and
their underpinning knowledge to manage complex interventions.
Teaches and advises patients and
their families on how to manage their condition or support the
multi-disciplinary team to do so.
Participates in clinical audits
and research projects and implements changes as required, including the
development, and updating of practice protocols / guidelines and procedures
locally.
Works within national and local
protocols where these exist.
Recognises boundaries of their
practice and know when and to whom patients should be referred.
Using
the principles of delegation, delegates work to other members of the MDT and
take responsibility for the delegated activity as appropriate;
Demonstrates initiative and are creative in finding
solutions to problems;
Holds responsibility for team performance and service
delivery

Network is an NHS Collaboration of 10 member practices plus two practices that
we provide services for. The practices are rurally located and the PCN covers a
large geographical area.
Our teams work closely, sharing
expertise and resources to develop a variety of services. Our vision is to
continue to improve the quality of care that we provide in line with the needs
of our patient population.
Our PCN builds on the existing
services and enables a greater provision of proactive, personalised and more
integrated health and social care. We are supported by practitioners who allow
us to create bespoke multi-disciplinary teams based on the needs of our local
population. By working together with local community services, this allows us
to make support available to people where it is most needed.

- Ability to work with independently
- Ability to organise and prioritise workload effectively
- Ability to exercise sound judgements when faced with conflicting pressures.
- Ability to handle a busy and varied primary care workload;
- Ability to offer support within a clinical team and share learning points in team meetings

- Qualified from a degree in nursing or from a registered nurse degree apprenticeship (RNDA);
- Registered with the Nursing & Midwifery Council;
- Post graduate qualification at level 7 or above relevant to their area of enhanced practice, for example in wound care, diabetes plus respiratory and CVD, dementia, womens health and public health and population health management;
- Working at an enhanced level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework and
- Works across the PCN, in services that enhance the core general practice offer.
