Job summary

A vacancy has arisen to join our mature Primary Care Network serving the populations of our practices across West Northumberland, and employed through our Federation HPCA. The opportunity is to be part of an experienced team bringing the valued skills and knowledge of the Paramedic Team to the patients and practices across the Tyne Valley.

Paramedics work autonomously within the community at an advanced level of practice, using their enhanced clinical assessment and treatment skills, to provide the first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains and headaches. They are health professionals who practice at an advanced level having the capability to make sound judgements in the absence of full information and to manage varying degrees of risk when there are complex, competing or ambiguous information or uncertainty. PCN paramedics will provide a Home Visiting Service across the Network (acute visits and long-term conditions reviews) as well as acute clinics in practice when required.

Main duties of the job

Each PCN Paramedic is assigned up to 2 practices, but will regularly support other paramedics and practices to ensure that the needs of the practices and patients are met.

  • The PCN Paramedic will be an experienced clinician who is a qualified Paramedic and comfortable with working autonomously to provide care for the presenting patient, working alongside the multi-disciplinary team, and referring cases on to GPs and other health professionals as necessary.
  • The role will involve the assessment, diagnosis, treatment and referral of patients presenting with undifferentiated and undiagnosed conditions.
  • Provide a mobile and flexible home visiting service to patients.
  • Provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. This may be face to face, telephone or via electronic consultation methods.
  • Home Visits for those patients who require it for their Long term condition review
  • In-house practice clinics for patients with Acute conditions as required

Through our PCN we are bringing general practices and local health and wellbeing providers together to work at scale to provide a wider range of services to our population and to integrate with the wider health and care system.

As a PCN working with our health and social care partners, our multidisciplinary teams work closely together, supporting each other deliver the highest levels of care to meet the needs of our population.

  • Educated to Degree/Diploma level in Paramedicine
  • Health and Care Professions Council (HCPC) Registration
  • Completed 2 year 'Consolidation of Learning' Period as a 'newly qualified paramedic'
  • Able to operate at an advanced level of practice

Desirable

  • Non Medical Prescribing Qualification

Experience

Essential

  • working at masters level or equivalent capability in paramedic areas of practice or equivalent (such as advanced assessment diagnosis and treatment)
  • experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans
  • has a further three years experience as a band 6 (or equivalent) paramedic
  • working knowledge of Microsoft and GP practice and prescribing data monitoring systems
  • ability to write comprehensive clinical notes, implement and evaluate care plans
  • be aware of data protection (GDPR) and confidentiality issues particularly within a PCN

  • cognitive behavioural and motivational interviewing approaches / skills
