Job summary
The post holder will be a pharmacist, who acts within their professional boundaries, supporting and working alongside practice teams within the Beckenham PCN and will be expected to move between sites of all 6 practices in the Beckenham PCN. They will be part of a team of 3 existing Clinical Pharmacists.
The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
Main duties of the job
Post holder is an Independent Prescriber.
The post holder will work as part
of a multi-disciplinary team in a patient facing role. The post holder will
take responsibility for areas of chronic disease management within the
Beckenham PCN and undertake clinical medication reviews to proactively manage
patients with complex polypharmacy.
The post holder will coordinate
and support practices within the PCN in the network QI requirements and also
feedback the information to the Primary Care Network at meetings. They should also
provide primary support to general practice staff with regards to prescription
and medication queries. They will help support the repeat prescriptions system,
deal with acute prescription requests, and medicines reconciliation on transfer
of care and systems for safer prescribing, providing expertise in clinical
medicines advice.
The post holder will provide
clinical leadership on medicines optimisation, quality improvement and manage
some aspects of the quality and outcomes framework and enhanced services.
About us
Beckenham Primary Care Network (PCN) comprises 6 local
practices with a total population of around 60,000 registered patients. We are
looking to expand the team to include pharmacists working across the PCN. This
role will work closely with our expanding PCN team which includes community
pharmacists, deiticians, nurse associate and social prescribing in addition to the core practice teams.

degree in pharmacy and registration with the General Pharmaceutical
Council.
- Have a minimum of 2 years
experience as a pharmacist, demonstrated within a practice portfolio.

the breadth of common acute and long-term conditions that are likely to be
seen in a general medical practice.
- Hold an independent prescribing
qualification, or will be working towards an independent prescribing
qualification.
- Have the ability to plan,

conditions.
- Recognise priorities when
problem-solving and identifies deviations from normal pattern and is able
