Job summary

We are seeking a dedicated and compassionate Practice Nurse to join our healthcare team. The ideal candidate will play a vital role in delivering high-quality patient care, supporting the practice staff, and ensuring that patients receive the best possible treatment. This position requires a strong understanding of the practice nursing duties and a commitment to providing exceptional care in a clinical setting.

Main duties of the job

Duties

  • Conduct patient assessments, including taking medical histories and performing physical examinations.
  • Cervical Screening, Child and Adult immunisations, Long Term Condition Management (Asthma, COPD, Diabetes, Mental Health Physical Reviews, ECG)
  • Administer vaccinations, injections, and other medications as prescribed by physicians.
  • Provide patient education on health management, disease prevention, and lifestyle modifications.
  • Maintain accurate patient records and documentation in accordance with healthcare regulations.
  • Collaborate with doctors and other healthcare professionals to develop and implement patient care plans. Attend multidisciplinary meetings as required.
  • Monitor patients' progress and arrange any follow up care or assessments as required.
  • Ensure compliance with health and safety and infection control regulations within the practice environment

About us

Beacon Medical Centre is based in Higher Blackley. We have a friendly multi-disciplinary team, including GPs, Clinical Pharmacists, a Nurse and HCAs. We have a list size of 4500 and are a high QOF achieving practice.

JOB DESCRIPTION SUMMARY

  • Responsible for the delivery of practice nursing care to the practice population.
  • They will deliver evidence based care for patients presenting with long term conditions and provide preventative health care to the practice population.
  • They will raise awareness among the population of the symptoms of long term conditions.
  • They will be committed to identifying and reducing barriers to presentation to primary care services.
  • As an autonomous practitioner, the nurse is responsible for the care delivered, demonstrating clinical decision making skills in the management of patients.
  • They will work collaboratively within the practice team to meet the needs of patients, support the delivery of policy and procedure, and provide nurse leadership as required.
  • Assess, plan, develop, implement and evaluate programmes which promote health and wellbeing and prevent adverse effects on health and well being
  • Assess, plan, develop, implement and evaluate individual treatment plans for patient with a known long term condition.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition as appropriate.
  • Work with other health care professionals to diagnose, monitor, manage and treat long term conditions, including nondrug based treatment methods using a management plan, and in-line with national and local policies and practice needs
