Job summary

To provide high quality, effective, evidence-based nursing care to meet the needs of our registered practice population.

The post is recognised as a development post for registered Nurses moving into Practice Nursing. Successful candidates will not be expected to undertake the full range of duties without support and training.

The post will focus on the pro-active management of long-term conditions as well as assessment, screening, preventative care and audit. The postholder will be a key member of our Nursing team and multi-disciplinary Practice team.

Main duties of the job

1. To communicate effectively with patients, carers and all members of the Practice Multi-disciplinary team and to maintain a caring environment through the support of colleagues.

2. To ensure accurate medical records are maintained at all times, which are consistent with legislation, policies and procedures.

3. To be responsible for the management of equipment and resources as part of the Nursing Team. This may involve for example, the ordering and maintenance of equipment / instruments / disposables, storage of vaccinations, drugs and other clinical stock, and ensuring that medical devices are calibrated, PAT tested and serviced where appropriate.

4. To support education and training for other staff members, medical students and others as required.

5. To provide holiday cover for other practice nurses as required

6. To attend staff meetings as requested.

7. To take care of your own safety and that of other people who may be affected by your own act or omission.

About us

Barrowford Surgery Ltd is made up of four clinicians and five administrative staff.We are also apart of Pendle East Primary Care Network and we work closely together the other practices. Throughthe PCN, we work with the ARRS staff that are employed.

Although the surgery is small, looking after around 3,500 patients, it can still be very busy. We support and help each other to try and ensure that we not only offer an excellent service to our patients but have a friendly caring environment in which to work.

Job responsibilities

The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

a. To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the practice manager

b. To support the production of the IPC annual statement in conjunction with the practice manager

c. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice

d. To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice

e. To develop, implement and embed health promotion and wellbeing programmes

f. To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing

g. To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health

h. As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF

i. As required, to diagnose and manage acute and chronic conditions

j. To prescribe medication for patients in accordance with PGDs and own scope of practice

k. To review patient medications to enhance compliance

l. To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages

m. To request pathology services and process pathology results as required

n. To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

o. To maintain accurate clinical records in conjunction with extant legislation

p. To ensure read codes are used effectively by all members of the nursing team

r. To develop, implement, embed and participate in well woman clinics
