We are a friendly, supportive, forward-thinking team in a busy town centre practice caring for patients with an array of interesting medical problems.
Main duties of the job
A key member of the clinical team, the successful candidate must hold a recognised Nurse Practitioner qualification. You will manage a caseload, dealing with a wide range of presenting healthcare problems and needs. The Advanced Nurse Practitioner will see both self-referred and internally referred cases and make professional autonomous decisions in relation to their problems, but remain fully supported by the existing clinicians. The post holder must be an Independent Prescriber.
About us
At St Vincent Practice, we aim to treat all our patients promptly, courteously and in complete confidence. Our team includes three GP partners, three salaried GPs and two Advanced Nurse Practitioners. We have a team of Practice Nurses and Healthcare Assistants who work alongside our Reception team and Admin/Secretarial team. We offer a full general practice service and run specialist clinics for children, diabetes, heart disease, COPD and asthma sufferers and for patients needing minor surgery.

Clinical Care
Act as Advanced Nurse Practitioner, demonstrating advanced clinical competence and a knowledge base beyond those associated with conventional nursing roles. Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.
Undertake consultations with patients as an autonomous practitioner and using own clinical judgement to diagnose, treat or refer patients
- Diagnose and manage both acute and chronic conditions, integrating both drug- and nondrug- based treatment methods into a management plan
- Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols.
- Work with patients and their carers in order to support compliance with and adherence to prescribed treatments
- Ensure care plans are in place, including emergency health care plans following discussion with the patient and family members
