Advanced Nurse Practitioner - frailty & complex care

Job summary

We are seeking a dedicated and skilled Advanced Nurse Practitioner to join our Community Advanced Nurse Practitioner Team delivering care to frail patients within their own homes and within residential and nursing homes. The post holder will have proven development of knowledge and skills within the realms of Advanced Practice in the care of people with long term needs with varying complexity including long term condition management, management of increasing frailty, dementia and end of life care. Consideration will also be given to those with proven Advanced Practitioner skills willing to further develop professionally within older people's care and frailty. As an Advanced Practitioner the post holder will be supported within their personal and professional development with increasing autonomy in professional clinical decision making as they utilise comprehensive assessment and care management skills to meet the acute and ongoing needs presented always acting within their own scope of professional practice.

Main duties of the job

  • Work autonomously as a valued and vital part of our well established Advanced Nurse Practitioner community team ensuring good communication and team efficiency in meeting the on the day and longer term workload demands.
  • To assess, diagnose, plan, implement and evaluate interventions and treatments for patients within your scope of professional practice.
  • Support transfer of patient's between their own home, acute / community hospitals ensuring there is continuity of care at admission and post discharge.
  • Autonomous clinical decision maker on a range of complex issues formulating plans of care that are safe, appropriate, including referral to acute services where required.
  • Ensure patients and carers / advocates views are taken into account with sound documentation to clearly understand how decisions were made.
  • Manage and respond to an array of clinical documents within a timeframe appropriate to need initiating onward referral, investigation, prescription or liaison with other specialists as required.
  • Be able to identify clinical emergencies, deteriorating patients or patients at risk of deterioration ensuring prioritising timely and appropriate actions to promote quality and safety of care delivery.

About us

Westcroft GP surgery host this role currently on behalf of Haven PCN. Haven primary care network is one of two primary care networks within Copeland ICC. The post holder will work on behalf of Haven PCN serving registered patients with Westcroft, Mansion House and Distington GP practices who are within a registered care home or at home living with frailty.

This post will add additional resource to what is currently available to support care home and community frail patients across the PCN. Currently we have the ANP Care Home Team of which this post will be a part and a frailty team both teams work closely with frailty care coordinators and other PCN staff including paramedics and pharmacy team. The PCN board recognise the value of proactive, personalised care for this patient group, frailty being one of our PCN priorities, hence support for this extra role when resource became available.
