Job summary

Vine Medical Group is recruiting for a full or part time Advanced Nurse Practitioner to join our Same Day Care Team.

Our new team member will work both independently and within a multi-disciplinary team environment, to provide safe, holistic care to our patients. This will include remote and face to face appointments, reactive and proactive home care visits. You will be required to work within your professional competence and boundaries to provide both urgent and planned care to patients. You will need to prioritise and triage the needs of patients, making any necessary referrals for investigations or to secondary care in the appropriate manner.

This role is particularly suited to an autonomous clinician with a proven track record of delivering high quality care within the Primary Care setting with experience in dealing with a wide range of minor ailments.

Job description

Job responsibilities

  • To provide a first point of contact within the service for patients presenting with undifferentiated physical and mental health conditions, offering holistic assessment. To include history taking, physical assessment and treating minor illness and ailments, undertaking investigations to inform clinical decision-making to establish a diagnosis and management plan in collaboration with the patient and their families or carers.
  • To request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc
  • Work within clinical boundaries, demonstrating safe and competent clinical decision-making, delivering high quality clinical care and maintaining patient confidentiality at all times
  • Provide telephone or video consultations, including the safe and effective triage of acutely unwell patients
  • Conduct reactive home visits to those acutely unwell patients, assess, plan, treat and arrange admission to hospital if required, for the practice and within the PCN
  • Future intention to conduct proactive home visits to housebound patients with long term conditions, including but not exclusively dementia, diabetes, respiratory disease.
  • Make appropriate referrals to primary, secondary and social care services, following practice (and/or locally agreed) pathways and protocols.
  • Where appropriately qualified:Supply, administer and prescribe medicines as indicated (and within competencies) to address patient need, in accordance with medicines management policy, patient group or patient specific directives or NMC standards

  • Promote self-care and educate service users to allow them to be empowered to make informed choices about treatment
  • To appropriately request and collect pathological & microbiology specimens including venepuncture, swabs, urine culture in line with local guidelines and process results as required actioning them in a timely manner in line with protocols
  • Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health, promote patients to live healthily and encourage principles of self-care
  • Recognise and manage anaphylaxis, according to the current UK Resuscitation Council guidelines
  • Perform Cardio-pulmonary Resuscitation and the correct use of Automated Defibrillator following training, according to the current UK Resuscitation Council guidelines
  • To provide general health promotion education, advice on national screening programmes and guide/refer patients to appropriate services or other health professionals as deemed appropriate
  • Provide information and advice on prescribed or over-the-counter medication, on medication regimens, side effects and interactions as appropriate and within your level of competency
  • Support the clinical team with all safeguarding matters, in accordance with local and national policies
  • Accurate recording of care provision onto patient records ensuring correct coding of all procedures and information using approved methods and templates
  • Chaperoning and assisting patients where appropriate who are being examined by another clinician
  • Conduct routine and post-use inspections of emergency equipment and medications
  • Clearing and re-stocking own clinical room/trolley at end of the session ensuring stock rotation and date checking
  • Cleaning and maintaining clinical equipment, reporting faults promptly
  • Preparing and maintaining environments and equipment before, during and after patient care interventions
  • Actively participate in all aspects of infection prevention and control and champion this within the practice
  • Assisting with the collection and collation of data on needs related to health and well-being and support QOF requirements and data completion
  • Actively participate in seasonal and special projects as requested e.g. flu campaign
