Job summary
Vine Medical Group is recruiting for a full or part time Paramedic Practitioner to join our Same Day Care Team.
Our new team member will work both independently and within a multi-disciplinary team environment, to provide safe, holistic care to our patients. This will include remote and face to face appointments, as well as reactive home visits. You will be required to work within your professional competence and boundaries to provide both urgent and planned care to patients. You will need to prioritise and triage the needs of patients, making any necessary referrals for investigations or to secondary care in the appropriate manner.
This role is particularly suited to an autonomous clinician with a proven track record of delivering high quality care within the Primary Care setting with experience in dealing with a wide range of minor ailments.

Main duties of the job
To deliver high quality clinical care in a timely manner as appropriate to the patient needs.
To provide a first point of contact within the service for patients presenting with undifferentiated physical and mental health conditions, offering holistic assessment. To include history taking, physical assessment, undertaking investigations to inform clinical decision-making to establish a diagnosis and management plan in collaboration with the patient and the families or carers.

- This role involves working autonomously to a high standard of care within the same day care team triaging, assessing, diagnosing and treating minor illness and ailments as part of our on the day service, both on-site and at home.
- Supply, administer and prescribe medicines as indicated (and appropriate) to address patient need, according to medicines management policy, patient group or patient specific directions or NMC/HCPC standards.
- Provide telephone or video consultations, including the safe and effective triage of acutely unwell patients.
- Conduct reactive home visits to those acutely unwell patients, assess, plan, treat and arrange admission to hospital if required, for the practice.
- Provide advice to patients and their families or carers on general health care, lifestyle, advice, referring to internal or external services.
- Ensure clinical practice is safe, effective and evidence based and remains within boundaries of competence and acknowledges limitations
- Work within a team, be part of and to create a positive culture and atmosphere for patients and staff.
- Make appropriate referrals to primary, secondary and social care services, following practice (and/or locally agreed) pathways and protocols.
