We are looking to recruit an Advanced Clinical Practitioner to join our Primary Care Multi-Disciplinary Team.

Our Advanced Clinical Practitioner will provide clinical triage, face-to-face consultations and on occasion visit patients homes.

Greyfriars Surgery aims at all times to provide high quality holistic care, treatment and support to our registered population of just over 13,450 people: and our list size is ever increasing.

We are a training practice across a range of Registered Professions, 'apprenticeship programmes' and are very passionate about learning and education for all. We have excellent working relationships with Lincoln Medical School, other Universities, local colleges, the NHS Talent Academy and county-wide Training Hub.

We have a very close working relationships with both our Primary Care Network and Neighbourhood Team.

Main duties of the job

A key member of the Surgery integrated Primary Care Team.

An experienced Practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

You will work collaboratively with the primary care team to meet the needs of patients, supporting the delivery of policy and procedures, and supporting the training of GP Registrars, Clinical and other staff as required.

The successful applicant as appropriate to experience, skill and knowledge will be encouraged to take on 'lead roles' within the Surgery - to enable development of professional and personal interests.

Our team is lead by 3 experienced GP partners.

Included in our on-site team are 3 Salaried General Practitioners, Nurses, Nurse Associate, Health Care Support Workers, First Contact Practitioner, Clinical Pharmacists, Mental Health Practitioners, Midwives, Health Visitors, Advanced Physiotherapy Practitioners, Physiotherapists, Department of Work and Pension advisers, Chaplains, Care Co-Ordinators (Care Homes & Cancer) and an amazing Reception and Administration team.

The post holder is an experienced Practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the Surgery. The post holder will demonstrate critical thinking in the clinical decision-making process. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and supporting the training of GP Registrars and other nursing staff as required.

2. Key responsibilitiesClinical practice
  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
  • Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients
  • Diagnose and manage both acute and chronic conditions, integrating both drug-and non-drug- based treatment methods into a management plan
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
  • Work with patients in order to support compliance with and adherence to prescribed treatments.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
  • Assess, identify and refer patients presenting with mental health needs in accordance with the National Institute for Health and Care Excellence (NICE) guidelines and framework for Mental Health

  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment
