The Gardens Surgery are looking for an individual
to join our small and friendly team in the role
as care co-ordinator. We work together to deliver a high
standard of care for our patients and are looking for a team player
who will actively participate in promoting the care of our
patients and who is able to communicate with the team in an effective and
positive manner.
The successful candidate will ideally have experience
in a clinical or administrative role. Someone with an interest in
Health and Social Care would be ideal. You should be
organised, flexible and a good communicator. You should be motivated
and a compassionate individual.
Main duties of the job
Working alongside the Clinical Lead and Practice
Manager in recalling patients with long term conditions for yearly
review
Managing your workload as delegated and
reaching agreed targets.
Excellent verbal and written communication
skills
Passion for helping people and providing personalised
care
Strong leadership and management skills and the ability to
motivate others
An understanding of the organisations policies and regulations
Ability to maintain a high level of
confidentiality and respect for patients
Proficiency with computers and Microsoft office
tools such as Word, PowerPoint and Excel, knowledge of Emis would
be advantageous
Strong analytical skills and the ability to handle
multiple tasks at a time
About us
We are a well-established, reputable PMS GP practice
conveniently located next to Peckham Rye Park and within walking distance of
East Dulwich. Serving about 6,300 patients who are predominantly working-age
individuals and young families, we strive to maintain the best qualities of
traditional family medicine while also embracing innovation and ongoing
improvement.
Our team consists of three partners and four salaried GPs. We have a nurse, HCA and admin and support team. Attached to the practice are the PCN pharmacist, social prescribers, physiotherapist and mental health worker. We work closely as a team and support each other delivering a high standard of care to our patients.

Your role would be to:
- Proactively identify (using population data) patients with long-term conditions and support their personalised care needs e.g. preparing for a shared decision-making conversation or providing holistic support;
- Help patients manage their needs through answering queries, making and managing appointments, and ensuring that good quality written or verbal information is available to help with choices about care.
- Assist with patient queries, signposting, facilitating access to self-management education and support for patients to take more control of their health/wellbeing.
- Provide coordination and navigation for people and their carers across health and care services, working closely other primary care professionals; and
- Promoting and supporting PCN health initiatives such as immunisations, health checks, smears
- Proactively support call and recall process
- Working proactively with the PCN pharmacist to support medication reviews
- Excellent interpersonal skills and the ability to interact well with different kinds of people
- Assisting with the Appointment Book
- Provide administrative support to the practice.
- Working with the HCA to ensure that all clinical rooms are stocked and overseeing the ordering of items needed.
