Covering maternity leave for 1 year in the role of a Practice Nurse with Diabetic experience and knowledge, to review and support patients and surgery focusing on clinical excellence and meeting clear targets.

Main duties of the job

Undertake reviews for Diabetic Patients

Organise appointments and support HCA team with the diabetic review

Work with the Diabetic hub to ensure patietn support needs are met

We are a busy 17,200 patient GP Partner led surgery

Currently have over 1000 diabetic patients

Large clincial staff and support team
