Clinical Pharmacist

Job summary

We would like to recruit an experienced pharmacist who can prescribe but we will consider applications from pharmacists who are in the process of completing their prescribing training or willing to undertake prescribing course.

This vacancy is 30 hours per week, over five days, Monday to Friday, 9am until 3.30pm with a 30 minute unpaid break.

Main duties of the job

To support the practice in all aspects of medicines management, facilitate cost-effectiveness and safe prescribing.

1. Undertake medication reviews with patients.

2. Handle prescribing queries from clinicians and review specific patients clinical conditions, including diagnosis, review therapeutic monitoring and prescribing as agreed with the clinicians at the practice.

3. To liaise with pharmacists in hospital and community practice and with all other healthcare professionals.

4. To reconcile medicines following hospital discharge and work with patients and community pharmacists.

5. To support patients in the effective use of medicines by providing medication review services as agreed with the practice.

6. To advise the practice in order to support safe, efficient repeat prescribing systems that reduce waste, improve efficiency.

7. To act as a source of medicines information for all the practice team and patients.

8. To support the practice in changing prescribing practice in response to local and national policy and targets.

9. Liaise with nursing home on current issues.

We are a forward thinking 6 partner, 4 salaried GP practice based in Kippax and Garforth with a dispensing branch practice in Monk Fryston. Our list size is 11,200

We are looking for an enthusiastic pharmacist to work within our practice team. We have well developed systems for dealing with discharge letters, medication reviews and prescribing issues. We have support from our local PCN pharmacist team. We have regular inhouse meetings and training to which our practice pharmacist is invited. We work from modern buildings alongside PCN and community staff eg First Contact Physiotherapist, Physicians Associates, Chronic Disease Nurses, Social Prescribers etc.

Role

To support the practice in all aspects of medicines management, facilitate cost-effectiveness and safe prescribing.

2. Handle prescribing queries from clinicians and review specific patients’ clinical conditions, including diagnosis, review therapeutic monitoring and prescribing as agreed with the clinicians at the practice. This may include using the clinical system and retrieving patient notes, to deal with queries from patients and stakeholders.

3. To liaise with pharmacists in hospital and community practice and with all other healthcare professionals to ensure seamless and efficient patient care including the promotion of EPS and repeat dispensing.

4. To reconcile medicines following hospital discharge and work with patients and community pharmacists to ensure patients receive the medicines they need post discharge.

5. To support patients in the effective use of medicines by providing medication review services as agreed with the practice e.g. to those on 5/6 medicines which can be discussed with the pharmacist.

6. To advise the practice in order to support safe, efficient repeat prescribing systems that reduce waste, improve efficiency and help patients get the most out of their medicines.

7. To act as a source of medicines information for all the practice team and patients (e.g. around doses, side effects, adverse events, possible alternatives around out of stocks)

8. To support the practice in changing prescribing practice in response to local and national policy and targets e.g. NSF targets NICE guidance local formulary decisions, top tips and guidance.

9. To promote, agree and implement local and national prescribing strategies within GP practice e.g drug changes, MHRA safety alerts, NICE guidance.

10. To be responsible for managing own time, working alone and planning and organising workloads or projects, including dealing with tasks and requests in a timely manner.

11. To ensure regular updating by self-directive learning and attendance at courses that would help in personal and service development. Undertake CPD as per GPhC requirements.

12. Liaise with the local pharmacies on current issues
