PCN Clinical Pharmacist

Job summary

The post holder will lead the medications optimisation work for the S&W Herefordshire PCN. This role will take ownership for the PCNs successful achievement of all contractual requirements and will report on progress against these targets to the PCN Board. In addition to working within the scope of their clinical competencies in direct patient care, the role will act to coordinate the pharmacists and pharmacist technicians across the network to ensure that all practice-based roles have the information and skills required to successfully undertake the required medications work within the PCN.

Main duties of the job

The role will be varied and will work across a range of different activities and clinical conditions, including:

Oversight of the medications elements of the PCNs national Direct Enhanced Service (DES) contract and any local medications contracts

Undertaking structured medication reviews across designated cohorts of patients. These will include but not be limited to care home residents, patients on 10+ medications, patients on potentially addictive medication, patients over 85-years and patients at risk of drug interaction.

Management of long-term conditions, with a particular focus on cardiovascular disease management

Reviewing systems for safer prescribing

Contributing to repeat prescription authorisations and reauthorisation

Actioning acute prescription requests

Addressing both the public health and social care needs of patients

Undertaking clinical audit

The post holder will be supported by a GP clinical mentor at Alton Street Surgery and the Clinical Director of the PCN. They will also engage with pharmacy colleagues across Herefordshire for peer support.

The Pharmacist will ensure that PCN Practices integrate with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare, and help manage workload. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.

South & West Herefordshire PCN is made up of six practices centred around the market town of Ross-on-Wye: Alton Street Surgery, Pendeen Surgery, Fownhope Surgery, Much Birch Surgery, Golden Valley Practice and The Surgery Kingstone. The network covers a wide geographic area offering traditional General Practice support to our patients centred around the communities they live in. The position will be hosted by Alton Street Surgery as the employer.

Job responsibilities

Leadership and management

You will be expected to lead on all medications optimization work the PCN. This will require you to:

Understand of all core PCN contracts, the role of pharmacy in the contracts and how to set goals and work with practices to achieve the required targets

Lead the pharmacy function across the PCN. This will involve motivating colleagues across the network to work collaboratively, whether employed by the PCN or whether colleagues working directly for their GP surgeries.

Demonstrate the ability to identify and resolve risk management issues according to policy/protocol

Work with PCN Board to review the role of pharmacy team in the management of long-term conditions

Clinical Medication Review

Undertake clinical medication reviews by phone or video with patients and work within your scope of practice as an independent prescriber to implement any necessary changes (or produce recommendations for/refer to other prescribing pharmacists, nurses and/or GPs to implement if outside your scope). These reviews could be cohort-based, for care homes, polypharmacy or any other area required by the PCN, within the pharmacists competence.

Medicines quality improvement

Undertake clinical audits of prescribing in areas identified by yourself and agreed by the PCN or as directed by the PCN, feedback the results and implement changes in conjunction with the relevant practice team.

Identify cohorts of patients at high risk of harm from. This might include risks that are patient related, medicine related, or both. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups.

Medicines safety

Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

Management of common/minor/self-limiting ailments

Managing caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence. Signposting to community pharmacy and referring to GPs or other healthcare professionals where appropriate.

Management of medicines at change of care setting

Reconcile medicines following discharge from hospital or admission to intermediate care or care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Set up and manage systems to ensure continuity of medicines supply to high-risk groups of patients (e.g. those with medicine compliance aids or those in Care Homes).

Medicine information to practice staff and patients

Answer relevant medicine-related enquiries from GPs, other network staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines. Suggest and recommend solutions. Provide follow up for patients to monitor the effect of any changes.

Signposting

Ensure that patients are referred to the appropriate healthcare professional for the appropriate level of care within an appropriate period of time e.g. pathology results, common/minor ailments, acute conditions, long term condition reviews etc.

Repeat prescribing

Ensure each practice in the PCN has a robust repeat prescribing policy, and streamline these across the PCN where possible. You may be asked to contribute to the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates. Ensure patients have appropriate monitoring in place when required.

Service development

Contribute pharmaceutical advice for the implementation and delivery of new services that have medicinal components (e.g. advice on treatment pathways and patient information leaflets).

Information management

Analyse, interpret and present medicines data to highlight issues and risks to support decision making.

Care Quality Commission

Work with the general practice teams to ensure the practices are compliant with CQC standards where medicines are involved.

Public health

Support public health campaigns. Provide specialist knowledge on all public health programmes available to the general public.
