Job summary
We are looking for a committed, motivated Practice Nurse to
join our experienced and friendly nursing team at Alton Street Surgery.
You will have a caring nature and a proactive professional
manner to provide a high standard of nursing care.
You will be working as part of a Nurse team with HCA support.
Main duties of the job
To be responsible for the delivery of evidence-based nursing
services, working as part of the practice multi-disciplinary team, delivering
care within their scope of practice to the entitled patient population.
Working autonomously, you will be responsible for a number of clinical
areas such as health promotion, chronic disease management and health prevention.

practice. The practice provides high quality care to approximately 11,550
patients in Ross-on-Wye and surrounding rural areas.
We
have a young and enthusiastic Partnership who are family friendly and who
encourage a strong team ethos within the surgery. It is very important to the
partners that this is maintained and developed. Learning is important in the
organisation, and the partners encourage the development of staff.
The
practice has the support of a large clinical team, which includes allied health
professionals such as an ANP, ACP, Clinical Pharmacist and an experienced
nursing and HCA team. We have dedicated administration and reception teams who
actively manage workflow optimisation, signposting, and pathology delegation,
helping to reduce some of the administrative burdens.
We
are a training practice (with 4 GP trainers) that also provides medical cover
to the local 32 bed community hospital opposite the practice, and an active
member of the South and West Herefordshire Primary Care Network.

Job responsibilities
CLINICAL
- To provide and maintain high standards of evidence-based patient care and work with practice policies and procedures.
- To co-operate and participate in practice audit and research as appropriate.
- To maintain accurate and comprehensive nursing records including computerised information, in accordance with practice needs, policies and procedures.
- To work within the NMC Code and adhere to its principals.
- Assist in and perform routine tasks related to patient care as directed by GPs
- Removal of Sutures
- ECGs
- Venepuncture
- Cytology
- Ear Care including Microsuction
- Sexual health screening
- Childhood Immunisations and national vaccination programmes (eg flu, covid)
- Travel Immunisations, education and administration
- Management of patients with Chronic Diseases
- Contraception monitoring including giving advice
- Following agreed clinical protocols with referral to Nurse Manager or GPs as appropriate
- Chaperoning and assisting patients where appropriate who are being examined by another clinician
- Assisting GPs with minor surgery & Sexual Health Clinics (IUDs insertion & removal)
- Requesting pathology tests, for example urine culture, swabs,
- Health Promotion work, e.g. weight management and smoking cessation.
- Safeguarding awareness
- Support the IPC Lead in Infection Control & Risk Management
- Maintaining and cleaning equipment used by the nurses and GPs
- Maintaining GP and Nurses rooms, stocking and rotating items as required
- Maintain general tidiness and cleanliness of nurses and treatment rooms
ADMINISTRATIVE
- Ensure accurate notes of all consultations and treatments are recorded in patients notes on the clinical computer system as appropriate.
- Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice.
- Ensure collection and maintenance of statistical information required for regular and ad hoc reports.
- Assist in the formulation of Practice Clinical Action Plan and Policies and develop appropriate protocols.
- Ordering of Health Promotion Literature
- Oversee the ordering of stock from various suppliers
- Ordering of vaccinations to maintain stock levels
- Participation in administrative systems in the practice
- A duty to advise Practice Manager of potential problems or errors within the range of assigned tasks
- Any other delegated duties appropriate to the post.
TEAM MEMBER
- To establish effective working relationships with colleagues and other agencies concerned with patient care to ensure a high standard of individual patient care is co- ordinated and maintained.
- Maintain excellent communication skills with all team members
- Work effectively as a team member
- Attend meetings relevant to post including Team, Clinical, MDT meetings
- Maintain an awareness of own role (including its limitations) and not to undertake duties outside the responsibility of a Practice Nurse and the NMC Code.
