Job summary
North Halifax Primary Care Network is looking to recruit an innovative, motivated and enthusiastic MSK First Contact Physiotherapist to expand our well established NHPCN team. We are looking for someone who is keen to develop within the team, be mentored by our experienced clinicians and undertake training with clinical supervision whilst contributing to the roles and workstreams outlined.
North Halifax PCN is a well established network of 4 practices who have worked collaboratively delivering care to our population of approximately 38,000 for more than 5 years.
Our MSK team along with other members of the PCN (practices, clinicians, clinical pharmacists, personalised care team etc.) aim to provide expert MSK advice in a variety of settings as detailed below.
We hope the person joining our team will be keen to develop in the role providing additional injection clinics, MDT type work with chronic pain and a prescribing role if these are already not part of your experience. We will provide the necessary supervision to complete training.
Main duties of the job
You will work as part of our hub-based approach which helps team morale, peer to peer support and effective management. It will also involve outreach spoke type clinics/visits at the member practices to build relationships and enhance the services at a very personal level as care closer to home is important to our population.
You will work independently but closely with North Halifax Primary Care Network and partnership teams to deliver service transformation and design with particular reference to your role.
You will ensure that the practice integrates with community and hospital MSK teams to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.
As part of your development, you will sign up to and complete the agreed road map and relevant training.
You will work as a self-sufficient practitioner and will be supported by the wider clinical teams. You must have excellent clinical skills, be proactive and self-motivated, but most importantly, have a passion for maintaining high standards of care and developing services with the patient at the centre of all that you do.
Whilst undertaking comprehensive assessment and diagnosis of patients with a wide variety of musculoskeletal needs, you will formulate, deliver, review and progress individual programmes and ensure effective treatment.

We are soon to move to new purpose built premises and this shows our commitment to providing services in the area . The new premises will allow for more expansion in the coming months and years.
We work very closely with our practices to ensure all process are standardised and closely aligned with individual practice needs , patient requirements and our own PCN DES requirements.
We have regular staff meetings , huddles , clinical and non clinical support and supervision , a digital lead ,and other teams as mentioned.
We have protected learning once a month that helps with CPD. We are always looking for ways to enhance our services and are open and welcome to new innovative ideas.
