We are seeking to employ a qualified and suitably experienced,prescribingClinical Pharmacist to enhance our team of 6 Clinical Pharmacists.

The position is 20 hours (Wednesday, Thursday, Friday) subject to 29 days holiday per year + 8 public holidays and NHS pension. Our core business hours are 08:00 - 18:30h Monday - Friday. Other benefits of the position include:

Overview of your organisation
North Halifax PCN is a well-established network of 4 practices who work collaboratively delivering care to our population of over 38,000 patients. We are are progressive PCN serving over 38,000 patients with embedded services, working in a hub-and-spoke model. Our multidisciplinary team comprises:

The team is managed by a PCN Manager and clinically lead by the the Clinical Director, representing the PCN board. We have adopted the Lead Practice model of PCN, which allows us to offer continuous service for NHS pension.

Job responsibilities
The
following describes the primary areas of responsibility for this role. The
clinical areas for delivery will be agreed after discussion with PCN leads and
in line with the individuals own scope of practice and experience. This may
change according to the DES requirements and PCN/practice/patient needs.
Patient Facing/Telephone Structured Medication Review - Undertake clinical medication reviews with patients with multimorbidity and polypharmacy and order relevant monitoring tests according to practice
protocols and policies. Review the on-going need for each medicine, review
monitoring needs and support patients with their medicines taking ensuring they
get the best use of their medicines (medicines optimisation).
Patients Facing
Medicines Support - Provide patient facing clinics for those with questions,
queries and concerns about their medicines in the practice.
Telephone Triage - Assist in
medication related triaging of patients as required by the practice.
Telephone Medicines Support - Provide a telephone help
line for patients with questions, queries and concerns about their medicines
with some bookable appointments.
Medicine Information to Practice Staff and Patients - Answer relevant medicine-related enquiries from GPs, other practice staff,
other healthcare teams (e.g. community pharmacy) and patients with queries
about medicines. Suggest and recommend solutions. Provide follow up
for patients to monitor the effect of any changes.
Work with case managers, multidisciplinary (health and social care)
review teams and hospital colleagues to manage medicinesrelated risk for readmission and patient harm. Put in place changes to reduce the
prescribing of these medicines to high-risk patient
groups.
Enhanced Health in Care Homes In line with the DES
lead/form part of the MDT working towards EHCH Care coordinator lead
Clinics As we expand the team, we are continuously
enhancing our services to our partner practices by using Population Health Management
approach, e.g. hypertension and diabetes management, lipid and other medicines optimisation
projects.
Management of
Medicines at Discharge from Hospital - To reconcile medicines
following discharge from hospitals, including identifying and rectifying
unexplained changes. Perform a clinical medication review of medical records,
produce a post-discharge medicines care plan including dose titration and
booking of follow up tests, and working with patients and community pharmacists
to ensure patients receive the medicines they need post-discharge. Set up and
manage systems to ensure continuity of medicines supply to highrisk groups of patients (e.g. those with medicine compliance aids).
Signposting -
Ensure that patients are referred to the appropriate healthcare professional
for the appropriate level of care within an appropriate period of time e.g.
pathology results, common/minor ailments, acute conditions, long term condition
reviews etc.
Repeat Prescribing
- Adhere to the repeat prescribing protocols.
Medicines Quality Improvement Programmes - Identify and provide leadership on areas of prescribing and medicines
optimisation. Conduct or work with other stakeholders on clinical audits and
improve projects. Present results and provide leadership on suggested changes.
Information
Management - Undertake clinical audits of prescribing in areas directed by the
PCN, feedback the results and implement changes in conjunction with the
practice's teams.
Medicines Safety - Adhere
to national and local policy
and guidance that affects patient
safety through the use of medicines, including MHRA alerts,
product withdrawals and emerging
evidence form clinical trials. Manage
the process of implementing changes to medicines and guidance for practitioners
and ensure compliance with medicines legislation.
Education and
Training - Provide education and training to primary healthcare team on
therapeutics and medicines optimisation. Provide training to visiting medical,
nursing, pharmacy and other healthcare students where appropriate.
Care Quality
Commission - Provide support to the general practice team to ensure the
practice is compliant with CQC standards where medicines are involved. Undertake
risk assessment and management and ensure compliance with medicines
legislation.

- Specialist knowledge acquired through post- graduate diploma level or equivalent training/experience
