Job summary
We are
looking for a registered and experienced Nursing Associate (NA) to support the organisation and facilitate clinical care in a holistic and proactive manner to patients within the PCN. We are looking for a a flexible, self-motivated person, willing to embrace change and be proactive in continuously improving service delivery.
The post for up to 20 hours per week. Although the main responsibilities follow the PCN Direct
Enhanced Service (DES), we envisage the right candidate will also help in
projects wider than the DES.
The hours of work will be be an agreement within the GMS core business hours 8:00 - 18:30h
The post
holder will work on a rota basis from our PCN hub in the new purpose built Mixenden
Community Hub and from practice premises, and for that the candidate will be
required to travel independently between
practice sites. Occasionally, there may be the need to perform house visits to patients in their homes.

- Another established workflow is to take part in Enhanced Health in Care Homes part of DES contract, which entails preparing and updating Personalised Care and Support Plans for care home residents.
- In addition, there may be a requirement to provide leadership and mentorship to trainee Nursing Associates within the PCN, and you will work with ourPCN PracticeNurses to support with the development, rollout and running of PCN wide projects and services.
