Job summary

We are looking for an individual to work FULL TIME 37.5 hours, who are enthusiastic and motivated to join our Personalised Care Team (PCT), already comprising: Care Co-ordinators and Social Prescriber Link Workers, in North Halifax Primary Care Network (PCN), who are keen to continue developing new services and enjoy a challenge.
