Job summary
We are
looking for an individual to work FULL TIME 37.5 hours, who are enthusiastic and motivated
to join our Personalised Care Team (PCT), already comprising: Care Co-ordinators and Social Prescriber Link Workers, in North Halifax
Primary Care Network (PCN), who are keen to continue developing new services
and enjoy a challenge. Although the main responsibilities follow the PCN Direct
Enhanced Service (DES), we envisage the right candidates will also help in
projects wider than the DES.
The hours of work are 10 am to 6pm.
The post
holder will work in a rota basis at our PCN hub in the new purpose built Mixenden
Community Hub. Occasionally there may be
the need to work from our practices premises and for that the candidate will be
required to travel independently between
practice sites.
NHS experience is not
essential as training can be provided. We are looking for people who can meet
the essential criteria and are willing to learn and develop into the role. Your

North Halifax PCN is a well-established network of 4 practices:
- Beechwood Medical Centre
- Caritas Group Practice
- Keighley Road Surgery
- Plane Trees Group Practice
who have worked collaboratively delivering care to our population of over 38,000 patients, even before the networks were created in 2019. The PCT, along with other members of the PCN (member practices, clinicians, first contact physiotherapists, clinical pharmacists) aim to provide the essential link between the health and social care needs of patients. We realise that we need to tackle any health and social inequalities to grow and mature accessible services to everyone.

This

PCN. All members of staff should be prepared to take on additional duties or
relinquish existing duties in order to maintain the efficient running of the
organisation. This list may be
amended, following consultation with the post-holder, to facilitate the
development of the role, the Network and the individual.
Main Duties:
- Undertake
responsibilities in line with the PCN DES and focus on the delivery of the PCN care model to
reflect local priorities, health inequalities and population health management
providing a point of contact to facilitate continuing care requirements
- Bring together all of a persons identified
care and support needs and what matters to them
- Provide coordination
and navigation for individuals and their carers across health and care
services, working closely with the PCN team
- Support the
coordination and delivery of multidisciplinary work streams within the PCN
- Build and maintain
relationships within the Network, including staff at member practices, to encourage
best practice and outcomes
- Work in partnership
with the PCN and practice teams to provide extra capacity, resource, and expertise to
support North Halifax patients preparing for and following-up clinical
interventions
- Work closely with other health and social care
professionals to identify and support patients making sure that access to
appropriate services is available to them and ensuring that their needs are met
- Proactively identify and work with NH PCN
patients to support their personalised care needs.
- Help people to manage their needs, answer
their queries and support them to make and attend appointments
- Ensure that people have good quality
information to help them make choices about their care
- Support people to understand their level of
knowledge, skills and confidence when engaging with their health and wellbeing
- Consult
with patients and family members to discuss their health needs where
appropriate

support to patients/family/care home on information about patient condition(s)
- Provide coordination and navigation for people
and their carers across health and care services, alongside working closely

until a resolution is achieved
