Job summary
Miles Platting, Newton Heath & Moston PCN would like to recruit a pharmacist to complement our current clinical workforce. These roles will be based within GP practices in the Primary Care Network.
Main duties of the job
Main Duty of the Job
The pharmacist will work as part of the multidisciplinary team in:
Assisting patients with medication related problems and queries
Providing support to general practice staff in relation to repeat prescriptions and medication queries
Working with the prescribing team on safety and quality of the practice prescribing
Medicines reconciliation including requests for new medication on (e.g. on discharge summaries/outpatient letters) and reviewing repeat medications for newly registered patients
Support with completing necessary training such as independent prescribing if not already qualified
Scope to further develop skills in areas of interest e.g. respiratory, diabetes, minor illness
Carry out structured medication reviews for all patient cohorts
Undertake clinical audit to support patient care and safety
Participate in contracts related work helping the practice to achieve targets for QOF, IIF and the prescribing incentive scheme
Assist with chronic disease diagnosis, management and reviews (e.g. hypertension, diabetes, CKD, hypercholesterolaemia) subject to experience and training
