Main duties of the job

Provide advanced mental health assessment, triage and ongoing management in a busy general practice setting, including structured reviews (e.g., SMI) and enhanced service antipsychotic monitoring.

Work alongside a fellow Mental Health ACP and dedicated Care Co-ordinator, and with the Quality Improvement Team to improve pathways, coding/recall and delivery of mental health targets including QOF.

Deliver safe, patient-centred care through face-to-face and remote consultations, using robust clinical decision-making, safeguarding awareness and effective onward referral.

Job responsibilities

Clinical assessment, triage and management

Act as an autonomous clinician, ensuring clinical practice is safe and effective and remains within boundaries of competence.

Provide face-to-face and telephone consultations for patients presenting with a wide range of mental health presentations, including undifferentiated and undiagnosed conditions.

Take a comprehensive history and undertake mental health assessment, including risk assessment and safeguarding considerations.

Use expert knowledge and clinical judgement to identify potential diagnoses and agree a management plan with the patient.

Provide mental health triage, prioritising and directing patients to the right care at the right time, including urgent escalation where indicated.

Refer patients for investigations and or appropriate specialist services where appropriate.

Make a diagnosis where appropriate and independently decide on and carry out treatment, including prescribing medicines where qualified.

Ensure continuity of care including follow-up consultations and planned reviews.

Assess and evaluate with patients the effectiveness of treatment and care provided, and make changes as needed.

Support patients to adopt health promotion strategies, promote healthy lifestyles and apply principles of self-care.

Work effectively with individuals in other agencies to meet patients needs and coordinate care with the wider MDT.

Antipsychotic medication reviews

Undertake antipsychotic medication reviews and monitoring in line with local enhanced service requirements and practice protocols.

Work with the Care Co-ordinator and wider team to ensure recalls, monitoring and follow- up are completed safely and in a timely way.

Quality improvement, QOF and pathway development

Work with the Quality Improvement Team to improve mental health pathways, templates, coding accuracy, registers and recall processes.

Support delivery and evidence of mental health targets including QOF, using good clinical record keeping and data discipline.

Participate in audit and service evaluation activity and contribute to shared learning across the practice.

Identify and escalate themes, risks and quality issues and contribute to practical improvements across HRH sites.

Administrative and professional responsibilities

Participate in the administrative and professional responsibilities of the Practice Team.

Ensure accurate, timely and legible notes of all consultations and treatments are recorded in the patients computer record.

Ensure accurate completion of all necessary documentation associated with patient care.

Attend and participate in practice meetings.

Participate in the maintenance of quality governance systems and processes.

Recognise and work within own competence and professional code of conduct.

Prioritise, organise and manage own workload in a manner that maintains and promotes quality.

Maintain continued education by attendance at courses and study days as appropriate for professional development.

Develop and maintain a Personal Learning Plan.

  • MSc in Advanced Practice or equivalent or working towards.
