Main duties of the job
Based within the Quality Improvement Team (QIT), you will play a pivotal role in helping the Practice deliver high quality, outcome-focused care. You will take ownership of allocated patient recall pathways and lead or support a range of clinics, including PSA monitoring, Diabetes Year of Care reviews, Severe Mental Illness (SMI) health checks, as well as contributing to joint heart failure review clinics.
This role offers a true blend of clinical and non-clinical responsibilities. Alongside data-led recall and quality improvement activity, you will deliver hands-on HCA-led care, carrying out elements of Diabetes Year of Care reviews and SMI physical health checks in line with national guidance and practice protocols.
You will also be an integral part of the teams ad-hoc clinics, which are developed in response to ongoing reviews of QOF targets and performance. Working closely with GPs, nurses, and the wider multidisciplinary team, you will help ensure timely patient reviews, accurate data capture, and effective engagement.
Through proactive collaboration and intelligent use of practice and patient-level data, youll help improve outcomes for people living with long-term conditions while supporting the Practice to achieve QOF and locally commissioned services and meet its contractual and performance objectives.
