Nurse Practitioner / Paramedic

Job summary

We are seeking to recruit an enthusiastic and experienced Nurse Practitioner/Paramedic Practitioner to join our well-established clinical team, to replace a Nurse Practitioner who is retiring. The ideal candidate is an experienced clinician who can provide autonomous care for registered patients of the practice. He or She should have excellent clinical skills and will be competent in assessing, diagnosing, planning, implementing, and evaluating treatment/interventions independently and autonomously

Previous experience in minor illness in a primary care health setting is essential. You must be able to demonstrate safe, clinical decision making for patients acting within professional boundaries.

You should demonstrate critical thinking in the clinical decision-making process. You should be able to work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and provide leadership as required.

In order to work at this level, NMC requirements for advanced practice must be met.

BSc Hons Nurse Practitioner Degree or equivalent qualification, or working towards

Evidence of on-going Personal Development

Ability to treat minor illnesses and ailments

Prescribing Qualification

The ability to work autonomously

Safe clinical decision making

Computer literacy

Support in attainment of annual QOF targets

Safe clinical decision making.

Home visits - Maximum 1 per day

Job responsibilities

JOB PURPOSE

To provide an additional / alternative service to patients with a range of acute conditions, covering a range of specialist areas.

To provide direct access for clients with undifferentiated and undiagnosed problems and make an assessment of their health care needs based on highly developed nursing knowledge and skills.

To admit or discharge clients from caseload and refer them to other health care professionals as appropriate.

To make professional clinical decisions within the role of an autonomous practitioner, applying evidence-based practice and help lead on provision of nurse access for acute clinical conditions.

To practice within the NMC Code of Professional Conduct and maintain accurate records.

To provide advice and accept referrals from other health professionals

To accept a share of the daily tasks that are allocated to GPs and NPs on rota system ie, on call duties, home visiting for acute minor ailments, acting on electronic lab results and hospital correspondence.

To prescribe from an agreed formulary and refer to GPs as necessary.

To work collaboratively with members of the PHCT.

To provide therapeutic interventions within a broader health promotion/public health strategy and enable clients to take action to meet their priorities for health.

To help provide specialist education and training regarding acute/chronic clinical conditions to other nursing staff and students.

To provide specialist advice to patients and relatives in relation to clinical conditions

DIMENSIONS

To provide specialist nursing services to the practice population, offering direct access to clients with a range of acute and chronic health problems.

To provide an extended nursing role to enhance service provision within the practice in response to primary care initiatives.

To promote and utilise the extended team, where possible empowering the patient to make the appropriate choice of service or practitioner for their needs

To provide specialist advice for members of the nursing and Primary Health Care Team.

KEY RESULT AREAS

To provide direct access for patients with undifferentiated and undiagnosed clinical conditions to the practice population.

To utilise specialist clinical skills to assess, diagnose and implement management of this specific group of patients incorporating evidence-based guidance, and encompassing relevant local policies.

To refer patients directly to other health care professionals as needed and collaborate with health professionals across primary and secondary care, utilising locally agreed guidelines.

To prescribe from the independent/supplementary nurse prescribing formulary and refer to GPs as necessary.

To communicate effectively with patients and carers utilising appropriate skills e.g. empathy, attentive listening.

Develop and provide autonomous nurse-led service to provide access for assessment and management of acute clinical conditions.

Assist with leading the Nursing Team in the planning and implementation of care provision for the practice population in support of the practice development plans.

Understand the importance of recognition, observation, documentation, communication and referral on of child abuse or family violence and be aware of local guidelines.

To provide health promotion advice opportunistically and specifically whilst recognising and utilising the cycle of change.

To provide a nurse telephone advice service for the practice population when required.

To assist with the development of clinical protocols for the nursing team.

To assist in clinical audit and the setting and monitoring of standards of care and implement standards for quality assurance.
