Job summary

We are delighted to be seeking a Senior Clinical Pharmacist to join our team in Beaconsfield. The role is mainly remote with 1 day every couple of weeks on-site. We are a family orientated 2 site practice supporting c18,500 patients. With a highly qualified team of 8 GP partners plus 4 salaried GP's we provide quality and caring personal health care service to our whole patient population. The team are supported by our nursing team, physio and paramedic services. The Senior Clinical Pharmacist role will play an integral part in ensuring the safe and effective use of medicines, resolving medication related issues, contributing to safe prescribing practice and improved health outcomes. Main duties of the job

As a key member of the practice team you will play a vital role in improving patients health outcomes. You will provide expert, timely and accessible medicines support to our patient population.

You will conduct structured medication reviews and manage long term conditions through medicines optimisation.

You will provide clinical advice and support to the wider healthcare team on complex medicines related queries.

The Simpson Centre relocated to a brand new purpose built site in 2022. We share the site with another practice but work independently in the main.Many of our team have been with the practice for 10+ years and we recognise people's commitment and motivation. As our Senior Clinical Pharmacist you are a key member of the practice team and will play a vital role in improving patients health outcomes. You will provide expert, timely and accessible medicines support to our patient population c18,500, mainly via telephone or Teams, with some face to face patient contact.

You will be competent in managing your workload and will implement an effective medicine management system within the practice, prescribing safely and effectively in the primary care setting.

Previous Senior Clinical Pharmacist experience is essential for this role.

Key Responsibilities

This job description covers the major areas of the Senior Clinical Pharmacists responsibility however the requirements of the role may vary and you will be expected to be flexible as and when required.

Conducting structured medication reviews to support safe, effective prescribing and improving patient outcomes

Managing long term conditions through medicines optimisation and tailored treatment plans

Providing clinical advice and support to the wider healthcare team on complex medicines related queries.

Acting as a key liaison for medicines reconciliation during care transitions, particularly following hospital discharge.

Supporting prescribing audits, medication safety initiatives and quality improvement projects.

Managing patients independently where appropriate and in line with professional qualifications and boundaries

Contribute to an integrated and patient centred model of care, ensuring medicines are used to their best effect

Carrying out for QOF - Asthma, COPD, Diabetes and hypertension reviews

Dealing with repeat prescriptions. Medication queries and discharge medications reconciliation

Admin work as necessary including reading and filing of letters in Docman

Regularly evaluate the delivery, implementation and risks involved with the service, contributing to clinical governance agendas and review of policies and procedures with ideas to develop these as required

Care Quality Commission

Maintain compliance with CQC regulations.
