Job summary

An exciting vacancy has arisen for a part time experienced practice nurse to work 3 days per week (Tues/Thurs/Fri), but with flexibility to cover annual leave when required.

Experience of working in general practice is essential.

Experience of using Systmone is preferable but full training will be given.

The role is varied and covers all aspects of practice nursing.

Main duties of the job

The candidate should have core knowledge and skills of management of long term conditions i.e. Diabetes, Asthma, Dementia etc Childhood Immunisations, Travel Vaccinations, wound management, INR, ABPI, smears and other enhanced services.

The Candidate should have the ability to work in a team, have good IT skills and strive to work for continuous improvement in their working role.

The candidate should be able to treat patients with dignity respect and compassion

We are a very friendly rural practice with a list size of approx. 5300. Great Staughton is located five miles from St Neots with excellent transport links to the A1/A14. Our team comprises one GP Partners, two SGP, one ANP, two PNs, one HCA and a very supportive patient services team. We are also a dispensing practice. We were rated first in Cambs and Peterborough for patient experience in the 2023 GP Patient Survey.

Job description

Job responsibilities

To be responsible for the delivery of practice nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice, the NMC code of conduct and professional boundaries to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management and health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

The following are the core responsibilities of this role:

a. To develop, implement and embed health promotion and well-being programmes

b. To implement and evaluate individual treatment plans for chronic disease patients

c. To identify, manage and support patients at risk of developing long term conditions, preventing adverse effects to patients health

d. To provide routine nursing care to patients as required in accordance with clinical based evidence, eg NICE

e. To provide wound care (ulcer/Doppler etc.) to patients

f. To provide travel medicine services

g. To request pathology services as necessary

h. To process pathology results as required

i. To provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required eg diabetes

j. To maintain accurate clinical records in conjunction with extant legislation

k. To ensure read codes are used effectively

l. To maintain chronic disease registers

m. To chaperone patients where necessary

n. To assist GPs with minor surgery when required ensuring consent received

o. To prioritise health issues and intervene appropriately

p. To support the team in dealing with clinical emergencies

q. To recognise, assess and refer patients presenting with mental health needs

r. To implement vaccination programmes for adults and children

s. To support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)

t. To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

u. To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)
