GP Practice Nurse

Job summary

We are looking for a GP Practice Nurse to carry out scheduled reviews of women who have diabetes and to focus discussions on preparing for a healthy pregnancy.

You will be employed by SDS MyHealthcare and work with the Sparkbrook Childrens Zone team.

Sparkbrook Childrens Zone has been operational since 2022 offering both childrens health and family support through our partnership with Early Help. Working closely with Early Help enables timely support for families for a variety of reasons; most commonly SEND needs/behavioural concerns, feeding issues and support accessing activities.

We can either employ on a locum/ad hoc basis or as a substantive member of staff.

Hours of work: 10h/week split between 2 days of the week; probably afternoons.

Location of work: GP surgeries within the Sparkhill, Balsall Heath & Moseley PCN.

This is a fixed term role end date 31 March 2028.

This is a pilot project so template and role will develop when we gain experience from service delivery.

Pay: £10,000 per annum as substantive employee OR £25ph as locum

Main duties of the job

Job role:

You will see women who have been invited to see you because they are age 18-45 and have either diabetes/prediabetes or a history of gestational diabetes. Our goal is to see women before they are pregnant, but at times you may see women who are pregnant. You will be guided by the Bump Ready toolkit as a basis for your consultation and will be expected to discuss/carry out these tasks:

o Optimising diabetes management

o Provide contraceptive advice

o Ad hoc immunisations if eligible (usually MMR preconception)

o Smears if within required recall

o Discuss wider determinates of health, referring to inhouse Early Help team.

o Appointments will be longer than usual (45 mins) to enable thorough discussions for women to learn about and take control of their health

Help us work together to increase the proportion of babies born healthy to healthy mums. You will have an energised team around you to help provide holistic care.

All practices use EMIS and you will use a template for standardisation of care

You will be supervised by an experienced GP.

This is a pilot project so template and role will develop when we gain experience from service delivery.
