Job summary

Under supervision of the GPs, the post holder will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills.

Main duties of the job

The post holder will demonstrate critical thinking in the decision-making process. They will work collaboratively with the healthcare team to meet the needs of the patients, supporting the delivery of policy and procedure. The post holder will provide a holistic and clinical service, with support and supervision of GPs, implementing agreed management plans and following provided protocols as appropriate.

Job responsibilities

South Doc Services are looking to recruit a full time PA.

Full time, 37.5 hours.

Salary: Dependent on experience

Clinical practice

Under supervision of the GPs make professional, autonomous decisions in relation to all treatment and preventative healthcare services in the Practice as delegated and agreed.

Provide direct clinical care to patients using established clinical guidelines.

Consult and refer patients to physicians, medical specialists and other health professionals as appropriate.

Conduct telephone consultations which may involve discussing the result and implications of laboratory investigations with patients as well as dealing appropriately with routine and emergency complaints.

Fully document all aspects of patient care and complete all required paperwork for legal and administrative purposes.

Contribute to the clinical development of the Practice by developing a special interest and to help establish appropriate systems to manage common chronic medical conditions, ensuring compliance with NSF guidelines.

Respond to medical problems presented by the patients, by telephone or face-to-face including history taking, examination, investigation, diagnosis, explanation of treatment and referral and management plan where appropriate.

Seek advice from other health professionals on matters beyond level of competence.

Response to requests for advice and support from other team members.

Act upon alerts and reminders during a routine consultation.

Provide appropriate preventative health care and advice.

Adhere to practice, local and national guidelines.

Follow up the care of patients as appropriate

Record clear and contemporaneous IT based consultation notes to agreed standards.

Deal with Practice notes, review and file patient lab. Results and discuss with GP where necessary.

Collect data for audit purposes.

Attend meetings as required

Undertake additional training where necessary to provide improved or new services.

Participate in the administrative and professional responsibilities of the Practice team.

Review test results and hospital referral letters

Review clinical and non-clinical data.

Participate in patient care planning

Review referrals, hospital admissions and A+E attendances.

Quality

Participate in the maintenance of quality governance systems and processes across the Practice.

In partnership with other members of the Practice team, collaborate on improving the quality of health care, responding to local and national policies and initiatives.

Support and participate in shared learning across the Practice

Personal and People Development

Take responsibility for own development, learning and performance including acting as a positive role model.

Understand own responsibilities and accountability in the delivery of services to patients, ensuring that the needs of the patient are the priority.

Contribute and participate in the development of local guidelines, protocols and standards.

Maintain effective communication with individuals and groups within the Practice environment and with external stakeholders.

Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
