Job summary

We are looking for a new team member who is passionate about delivering great care to our patients and who loves to work in a team.

Main duties of the job

The Nurse Practitioner (NP) is a professional health care provider who operates independently in a variety of settings, working as part of a team while making autonomous decisions about the care of patients, including history taking, clinical assessment, diagnosis, treatment and evaluation of their care.

The role combines advanced nursing skills with clinical diagnostic and treatment skills. The NP delivers primary health care services alongside General Practitioners (GPs) and Practice Nurses.

The NP will work collaboratively with the multidisciplinary general practice team to meet the needs of patients, supporting the delivery of excellent clinical care in accordance with our policies and procedures.

Job description

Job responsibilities

Clinical:

Assume a clinical workload caring for patients in our general practice population, in collaboration with GP colleagues.

Provide holistic, individualised healthcare, making appropriate use of both nursing and medical models of health and illness.

Demonstrate knowledge of the pathophysiology of acute and chronic conditions commonly seen in practice.

Accept referrals from other members of our multidisciplinary team.

Undertake the independent assessment of undifferentiated and undiagnosed problems with attention to physical, emotional, social, cultural and mental health issues

Use sound judgement in assessing conflicting priorities and needs.

Formulate a diagnosis utilising medical diagnostic problem-solving skills including history taking, physical examination, invasive and non-invasive procedures/investigations

Initiate appropriate diagnostic strategies and therapeutic interventions including laboratory requests, with attention to safety, cost effectiveness, acceptability and clinical efficacy.

Develop with each patient (and family) an evidence-based plan for long-term health, with an emphasis on preventative measures.

Interpret and evaluate diagnostic test results in liaison with the patient and their GP and initiate treatment accordingly.

Evaluate the patients condition and response to therapeutic interventions, and modify the plan of care accordingly for optimal outcomes.

Prescribe as an independent nurse prescriber in accordance with the legal framework for nurse prescribing and the Integrated Care System (ICS)s nurse prescribing policies.

Initiate independent and collaborative patient referral to statutory and non-statutory organisations including GPs; specialist consultants, other services within the ICS, allied health care professionals and voluntary agencies.

Make use of computer technology and automated information systems to support decision making, improve the quality and co-ordination of care and services and to enable faster and more certain communication within the ICS.

Maintain accurate records according to the Clinical Governance Framework and the Nursing and Midwifery Council (NMC) Guidelines.

Management:

Evaluate clinical work using audit cycles to effect improvement in standards of care and better planning of service delivery.

Engage in clinical supervision and reflective self-evaluation to improve practice and drive professional development.

Manage and adapt responsively to foreseen and unexpected changes in the work environment.

Communication:

Utilise advanced communication, negotiation and interpersonal skills in the interests of promoting patient-centred healthcare and effective teamwork.

Communicate the patients health status using appropriate terminology, format and technology. The course of this communication process involves the discussion of complicated issues and the sharing of delicate information on a regular basis.

Provide health promotion and disease prevention services to both patients who are healthy and who have acute and /or chronic conditions by offering anticipatory guidance and counselling to promote health, reduce risk factors and prevent disease and disability.

Recognise peoples needs for alternative methods of communication and respond accordingly.

Use networks for sharing relevant information in a safe and appropriate manner.

Work closely with the Practice Nurse Lead and Nursing team to ensure a supportive environment with good communication, sharing knowledge and skills to improve the effectiveness of our clinical team.

Confidentiality:

Keep strictly confidential all personal information about patients and their carers, as well as personal information about colleagues and business information relating to the practice.

Disclose confidential information only to authorised persons in accordance with Practice policies relating to confidentiality and in compliance with data protection legislation.

Report actual or potential breaches of data confidentiality to the Practice Manager.

Health & Safety

Promote and maintain a safe and secure work environment to benefit their own and others health, in accordance with the Practice Health & Safety Policy.

Comply with safety instructions

Complete mandatory training and make effective use of other relevant training to update knowledge and skills.

Use appropriate infection control procedures, ensuring work areas are maintained to be tidy, clean and free from hazards.

Report identified actual or potential risks to the Practice Manager.

Report significant events and incidents in a timely manner in accordance with practice policy.
