Job summary

We are looking for an experienced Practice Nurse to join our team in Greenford Ealing. This is a key role within our team and you will lead in all areas of nursing and be responsible for a number of key QOF and enhanced service targets. We are a friendly team and offer a supportive work environment.

About us

We work from a purpose built surgery, located in Greenford Ealing. We are on public transport routes but also have free parking on the roads by the practice. We back on to Ravenor Park which is a large green expanse and good for walking at lunchtimes. We have approx 6100 patients and work closely with other practices within our PCN. Ealing nurses are also supported with monthly forums which we would encourage you to be part of.

Our team is supportive and led by 4 GP Partners - we are open to flexible working and condensed hour working over a 4 - 5 day week.

We are seeking to recruit a Practice Nurse as our current practice nurse is leaving us after 8 years to work in secondary care. The role is 34 - 37 hours per week and we are flexible with hours. You would be part of our friendly team and supported by a HCA, Pharmacist and Phlebotomist as well as our wider team of GPs, Social Prescribers, Physios

Our Practice Nurse, supported by the admin and clinical team, leads in particular areas of QOF and enhanced services practice targets. We are a high performing practice, with an excellent reputation for delivering responsive and patient centred care.

Recent Practice Nurse experience is essential with a desire to deliver high quality care. You must be reliable, professional and friendly with a can do and flexible attitude to work. We are looking for someone who is an excellent communicator, possesses good IT skills, we use Systmone, and importantly, is able to work as part of a team.

Main duties of the job

The successful applicant will be required to provide the full spectrum of Practice Nurse duties including: Management of long term conditions (Diabetes, Asthma, COPD, Hypertension), wound care, sample taking, ECGs, cervical cytology, childhood and adult immunisations and travel vaccines.

Work as part of the Nursing team to assess, plan, provide and implement and evaluate nursing care to meet the care needs of individuals in the practice population

Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition

Assist in and perform routine tasks related to patient care as directed by GPs

Working alongside the GPs to manage Chronic Disease including Asthma, COPD, Diabetes, Heart Disease etc

The following are the core responsibilities of the Practice Nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Develop, implement and embed health promotion and well-being programmes

b. Work alongside patients to develop individual treatment plans for chronic disease

c. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

d. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF (inclusive of contraception and cytology)

e. Provide wound care to patients we do not offer Doppler at the surgery

f. Provide travel medicine services

g. Undertake services such as NHS Health Checks, ECGs, local enhanced or incentivised schemes

h. Request pathology services as necessary, processing and interpreting results as required

i. Refer patients directly to other services as appropriate

j. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required

k. Maintain accurate clinical records in conjunction with extant legislation

l. Ensure relevant clinical codes are used effectively

m. Maintain chronic disease registers

n. Develop, implement and embed well woman/man clinics

o. Chaperone patients where necessary

p. Assist GPs when required

q. Prioritise health issues and intervene appropriately
