We are seeking a motivated and organised Care Co-ordinator/Secretary to join our friendly and supportive General Practice team. This is a key role within the practice, supporting both clinical staff and patients to ensure high-quality, coordinated care
Main duties of the job
This role plays a key part in supporting patients, clinicians, and external healthcare providers to ensure safe, well-coordinated care. The successful candiate will act as a central point of contact, helping to manage communication, clinical correspondence, and referrals while supporting patients with long-term conditions and complex needs.
The role involves providing high-quality secretarial and administrative support to the clinical team, maintaining accurate patient records, and liaising with community services, hospitals, and other professionals to support multidisciplinary team (MDT) working and care planning. A strong focus on confidentiality, data quality, and information governance is essential at all times.

We are a well-established GP surgery serving approximately 6,000 patients within our local community. The practice is led by three GP partners who are committed to delivering high-quality, patient-centred care while fostering a supportive and progressive working environment for staff.

The role acts as a key point of contact between patients, clinicians, and external healthcare services, supporting patients with long-term conditions and complex needs.
Key Responsibilities

Act as a first point of contact for patients, clinicians, and external services regarding care coordination and clinical administration.

Support the coordination of care for patients with long-term conditions and complex needs, working closely with the multidisciplinary team (MDT).

Manage clinical correspondence including referrals, hospital letters, test results, reports, and onward actions.

Provide comprehensive secretarial support to clinicians, including document preparation, workflow management, and task prioritisation.

Maintain accurate, timely, and high-quality patient records in line with practice policies and information governance requirements.

Liaise effectively with community services, hospitals, social care, and other healthcare providers to support continuity of care.

Support MDT meetings, care planning processes, and follow-up actions as required.

Ensure confidentiality, data protection, and information governance standards are upheld at all times.
