Job summary

This is an exciting opportunity for a motivated and organised Care Coordinator to join our Primary Care Network (PCN) Wellbeing Team, supporting the delivery of high-quality, patient-centred care in the Falmouth and Penryn area.

Working closely with an established, supportive Wellbeing Team and wider multidisciplinary teams, you will play a key role in coordinating patient care, improving access to services, and ensuring a smooth and efficient patient journey. This is a varied role combining administration, communication, and service coordination, with a strong focus on supporting proactive and personalised care.

Our Wellbeing Team is highly valued and well-integrated within practices, with strong support from the wider PCN and partner organisations.

Main duties of the job

The role focuses on coordinating patient care and supporting the smooth day-to-day running of the Wellbeing Team. This includes managing administrative processes, organising and contributing to multidisciplinary team (MDT) meetings, maintaining accurate patient records, and helping track performance and outcomes.

You will act as a key point of contact, communicating with patients, clinicians, and partner organisations.The role also involves supporting care planning, facilitating collaboration across teams, and contributing to the delivery of high-quality, patient-centred wellbeing services.

Falmouth & Penryn PCN is a well-established, forward-thinking collaboration of four GP practices (Penryn Surgery, Falmouth Health Centre, Trescobeas Surgery and Westover Surgery) serving approximately 50,000 patients across central Cornwall.

At the Falmouth & Penryn PCN, we pride ourselves on being progressive, collaborative, and adaptive to the evolving needs of our population. We piloted the countys very first Mental Health Hub, which is now being adopted in many other parts of the county.

Our multidisciplinary team of approximately 30 colleagues includes Mental Health Practitioners, Clinical Pharmacists, First Contact Physiotherapists, Paramedics, Pharmacy Technicians, Community Health Wellbeing Coaches and Social Prescribers. This diverse clinical mix allows us to address a wide range of health needs and deliver high-quality, personalised care.

The PCN is led by a Clinical Director and supported by a dedicated team under a Lead Practice model. This structure ensures integration with NHS pension benefits.

Job responsibilities

Main Duties

  • Support the Wellbeing Team with day-to-day duties
  • To communicate effectively and sensitively and use language appropriate to professionals and patients and carer/relatives condition. Understand and be aware of and manage barriers to communication.
  • To manage the day-to-day operation of Wellbeing Team functions to maintain provision of high-quality services, supporting admin, room bookings, and completed reports when requested.
  • To coordinate and attend MDT meeting to take the meeting minutes.
  • Maintain accurate and up to date records of patient records on the clinical system
  • To run regular patient searches using the Clinical System in order to have an up-to-date record of progress of achievement of Key Performance Indicators.
  • Assist the Clinical Team Lead by managing administrative processes, scheduling, and coordination activities to support the delivery of high-quality wellbeing services.
  • To receive and make telephone calls from/to patients and other healthcare professionals, signposting to the appropriate clinician/service(s) to support the Wellbeing Team where there is increased demand.

Administrative Duties

  • Under guidance from the line manager, take initiative in the organisation and administration of all Wellbeing Team meetings including creating meeting agendas, taking minutes and sending out invites to the relevant staff members.
  • Manage agenda items, ensuring all new referrals and cases for discussion are identified and information circulated to team members in advance of the meetings.
  • To ensure that action points identified within the MDT are recorded and followed up

Other Responsibilities

  • Work closely with the Clinical Team Lead to provide administrative support that enables smooth coordination of wellbeing services and enhances patient care.
  • To play a role in the delivery of high-quality mental health primary health care services
  • Maintain a working relationship with local health care providers enable service delivery of mutual benefit and build a network and knowledge of referral routes to and from service providers.
  • To establish and maintain effective liaison with stakeholders including health, voluntary, social and education resources, attending relevant meetings as necessary
  • Collate feedback / analysis data on behalf of the Wellbeing Team to report to the PCN Board and CCG/NHSE as required
  • Be responsible for the organisation, planning and of own workload to meet set deadlines.
  • Following PCN and practice policies and procedures as appropriate.

Duties will vary from time to time under the direction of the Wellbeing Team Clinical Lead dependent on current and evolving PCN Wellbeing Team workload and staffing levels.
