Job summary

We are looking for a Practice Nurse to join our large supportive nursing team at Park Road Group Practice. Experienced in chronic disease management would be desirable but not essential.

Main duties of the job

The practice nurse, working as part of a multidisciplinary team will be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice. You will ensure nursing services are delivered effectively to our practice population. The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, chronic disease management, health promotion, wound management, as well as supporting the practice management team in the reviewing and delivery of clinical policies and procedures.

Job description

Job responsibilities

Duties which may be included in the role but not exhaustive:

Support the Infection Prevention Control lead and to work with our IPC Policy supporting the implementation of audits, training, support and evidence of compliance to the Clinical Manager.

Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice.

Develop, implement and embed health promotion and wellbeing programmes.

Implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing.

Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health.

Identify and manage acute and chronic conditions.

Manage wound care, including leg ulcer management, compression and doppler.

Prescribe medication for patients in accordance with PGDs and own scope of practice.

Review patient medications to enhance compliance, within own scope of practice.

Coordinate the provision of travel medicine services.

Request pathology services and process pathology results as required.

Oversee the provision of chronic disease, delivering patient care as necessary, referring patients to secondary/specialist care as required.

Ensure coding of medical records are used effectively.

Ensure the appropriate maintenance and use of chronic disease registers.

Develop, implement, embed and participate in well-being of patients.

Prioritise health issues and intervene appropriately.

Support the team in dealing with clinical emergencies.

Recognise, assess and refer patients presenting with mental health needs.

Implement vaccination programmes for adults and children.

Liaise with external services/agencies to ensure the patient is supported appropriately.

Delegate clinical responsibilities appropriately, ensuring safe practice and the task is within the scope of practice of the individual.

Support the clinical team with all safeguarding matters in accordance with local and national policies.

Monitor and ensure safe storage, rotation and disposal of vaccines, medicines and consumables.

Support junior members if the nursing team by providing support and guidance where necessary.

Support and participate in shared learning.

Participate in local initiatives to enhance service delivery and patient care.

Participate in the review of significant and near-miss events applying structured approach.
