Job summary
We are looking for a fixed-term
Advanced Clinical Practitioner to join our team. The role
would suit a practice Clinical Pharmacist with experience wanting a fixed-term role. Locums
considered.
The role is to cover maternity leave, and is available for a fixed term of 9 months (or less if the staff member on maternity leave returns early). The post is funded via the Additional Roles Reimbursement Scheme (ARRS).
Main duties of the job
This role involves providing expert Clinical Pharmacist care to patients assessing patients, prescribing and reviewing medications, providing education and support to patients. As well as this, you will be required to contribute towards national clinical targets including IIF as well as prescribing guidelines.

Key Responsibilities:
- Patient Assessment and Management - Conducting
patient assessments, including taking medical histories, performing physical
examinations, requesting relevant investigations, developing and implementing
management plans, and making necessary referrals to other members of the
Practice team. Manage undifferentiated undiagnosed conditions and identify red
flags and underlying serious pathology and take appropriate action
- Personalised Care use complex decision making
to inform the diagnosis, investigation, complete management of episodes of care
within a broad scope of practice, and actively take a personalised care
approach and population centre care approach to enable shared decision making
with the service user
- Medication Management - Prescribing and
reviewing medications for medicine optimisation of long-term conditions and
structured medication reviews.
- Medication Safety - Identifying and mitigating
risks associated with medication use for patients and the practice.
- Patient Education and Support - Providing
information and advice to patients on their medications, promoting adherence to
treatment plans, and supporting self-management strategies.
- Multidisciplinary Teamwork - Collaborating with
other healthcare professionals, such as doctors, nurses, and other allied
health professionals, to optimise patient care and ensure a seamless approach
to treatment. Attending weekly MDT meetings with our partners care home to
contribute and manage vulnerable patient health.
- Service Development and Improvement -
Participating in clinical audits and quality improvement initiatives including
IIF, QoF and Prescribing Incentive scheme. Working closely with the ICB to
ensure effective prescribing in line with local formulary.
- Leadership and Education - Providing leadership
to the Pharmacy Technicians and wider clinical team. Maintain and develop own
training and learning.
- Encompass the four pillars of clinical practice;
leadership and management, education and research, with demonstration of core
capabilities and area specific clinical competencies
