Job summary
Care Coordinators play an important role within our PCN working closely with our GP practice teams, Social Prescribing Link Workers and wider health, social care and community colleagues. They will identify and manage a caseload of identified patients, making sure that appropriate support is made available to them and their carers. Care coordinator will set up and co-ordinate MDT meetings and be the point of contact for the team. They will be involved with risk stratification, data searches, monitoring and evaluation of services
The successful candidates will have excellent organisational and communication skills, and will work with a diverse range of people and professionals, across different surgeries, community services and in the community.
Main duties of the job
The successful candidate will work with patients from across our four practices. They will be caring, dedicated, reliable and person focussed and enjoy working with a wide range of people. They will have good written and verbal communication skills and strong organisation and time-management skills. They will be highly motivated and proactive with a flexible attitude, keen to work and learn as part of a team and committed to providing people, their families and carers with high quality support.
The High Intensity User (HIU) Care Coordinator plays a pivotal role in proactively supporting patients who frequently present to primary and emergency care services due to complex medical, social, or psychological needs. The aim is to reduce avoidable hospital use, improve patient wellbeing, and enable more coordinated, person-centred care.
The HIU Care Coordinator will work closely with GPs, social prescribing link workers, mental health teams, social care, emergency departments, and voluntary sector organisations to create personalised support plans that help patients manage their health more effectively and reduce unplanned service use.
This is a non-clinical but essential role within the PCNs multidisciplinary team.

Rosebank Health PCN, includes Rosebank Surgery, Severnvale Surgery, Bartongate Surgery and Kingsway Health Centre. The successful candidate will be required to work across all four sites.

The PCN has a friendly multidisciplinary team which includes: GP's, Nurses, Paramedics, Pharmacists, Physiotherapist, Care Home Co-ordinator& Social Prescribers.
