Job summary

Do you want to make a difference by supporting patients in Primary Care with your skills as a Children and Young Person Social Prescriber?

We need a talented and empathetic individual with excellent communication skills to be part of a growing team, helping children and families by providing a social prescribing service for Rosebank Health.

We are looking for someone with relevant qualifications, skills and resilience to work with GPs and practice staff, helping children by addressing their needs in a non-medical way. This is not a clinical role, but one that supports patients to address and tackle issues that are having an adverse effect on their mental and physical health.

The successful person will work across our four surgeries.

Main duties of the job

We are looking for an outstanding individual who, like us, is passionate about improving the health and wellbeing of all.

This post empowers people to take control of their health and wellbeing by giving time to focus on what matters to me. The post will build trusting relationships with people, create a shared personalised care and support plan and connect them to community groups, VCSE organisations and services. They will also work with partners to provide support to community groups and VCSE organisations involved in social prescribing. The SPLW team will support patients of all ages, with this post having an added focus on children and young people, and work with the whole person to develop a practical and emotional support network.

You must be a good listener, have time for people and be committed to supporting local communities to care for each other. You should have experience of engaging and working positively with people, including children and young people, facing complex social and emotional challenges. You will have excellent communication and interpersonal skills and be able to evidence supporting people, community groups and local organisations. You will have experience of working with individuals from different cultures, disabilities, sexual orientations and ages, preferably within a health, social care or education setting.

Rosebank Health PCN, includes Rosebank Surgery, Severnvale Surgery, Bartongate Surgery and Kingsway Health Centre. The successful candidate will be required to work across all four sites.

We hare a PCN in our own right, which enables us to develop & shape the services we offer. The PCN is forward thinking and innovative, prioritising excellent patient care. The PCN has a friendly multidisciplinary team which includes: GP's, Nurses, Paramedics, Pharmacists, Physiotherapist, Care Home Co-ordinator & Social Prescribers.

Key responsibilities

The following are the core responsibilities of the PCN CYP social prescribing link worker (CYPSPLW). There may be on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:

a. To take referrals from a wide range of agencies, working with GP practices within primary care networks, pharmacies, multi-disciplinary teams, hospital discharge teams, allied health professionals, emergency services, job centres, social care services, housing associations, and voluntary, community and social enterprise (VCSE) organisations.

b. Work with vulnerable children, young people and families, within communities, to establish needs based on a whole family assessment and contribute to forming a robust action plan agreed with the family and other key agencies.

c. Assist children, young people and families to take decisions on what type of support they require that best meets their health and wellbeing needs. This will involve using a range of behavioural change techniques such as motivational interviewing; health coaching and strengths based approaches to determine their skills, interests and motivation.

d. Co-produce and implement strength-based Personal Development Plans & Family Plans based on the their priorities, interests, values and motivations including what they can expect from the groups, activities and services they are being connected to and what the they can do for themselves to improve their health and wellbeing.

e. Provide a range of family support interventions that support the agreed outcomes such as parenting skills, behavioural, anger management and social isolation.

f. Act as mediator between children and parents to stabilise and rebuild family relationships and create positive futures.

g. Co-ordinate the services of other specialist agencies as appropriate to meet the needs of the child, young person and family i.e. Team Around the Family and Team Around the Child meetings.

h. Act as the Social Prescribing Lead Professional where required for target groups identified as part of the Population Health Management and MDT meetings.

i. To manage and prioritise your own caseload in accordance with the needs, priorities and any urgent support required by individuals on the caseload.

j. To have a strong awareness and understanding of when it is appropriate or necessary to refer people back to other health professionals or agencies, when what the person needs is beyond the scope of this CYPSPLW role.

k. Support young people with traits associated with autism and ADHD. Including advice on referrals for diagnosis with NHS or Right to Choose providers.

l. To build relationships with key staff in GP practices within the local Primary Care Network (PCN), attending relevant meetings, becoming part of the wider network team, giving information and feedback on social prescribing.

m. To ensure that social prescribing referral codes are inputted to the clinical system and that the persons use of the NHS can be tracked, adhering to data protection legislation and data sharing agreements with the CCG.

n. To meet people on a one-to-one basis, making home visits where appropriate within organisations policies and procedures giving people time to tell their stories and focus on what matters to me and building trust with the person, providing non-judgemental support, respecting diversity and lifestyle choices. The role requires working from a strength-based approach focusing on a persons assets.

o. To support in the delivery of enhanced services, quality improvement and other service requirements on behalf of the PCN.

p. To participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through incidents and near-miss events
