The Hollies Medical Centre is looking for an energetic, knowledgeable and experienced Clinical Pharmacist to continue to develop the growing pharmacy team.
Main duties of the job
The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside GPs, nurses and the admin team in general practice. In this role they will be supported by a GP partner who will develop, manage and mentor them and be part of a network wide pharmacist forum.
The post holder will work as part of a multi-disciplinary team in a patient-facing role dealing with patient facing medication queries,will take responsibility for areas of long term chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practice staff with regards to prescription and medication queries, they will help support the repeat prescription system, deal with acute prescription requests, the post holder will also provide clinical leadership on medicines optimisation and quality improvement and manage aspects of the quality and outcomes framework and enhanced services.
The post holder will be involved in the Primary Care Network and oversee Primary Care Network pharmacist work, providing training and supervision within the clinical team. They will proactively contribute to practice developments, promoting new ways of working in their clinical field.

Interviews will be held over lunchtime Wednesday 25th October.
The post holder is a pharmacist, who
acts within their professional boundaries, supporting and working alongside
GPs, nurses and the admin team in general practice. In this role they will be

ailments and signposting to community pharmacy or other healthcare
professionals where appropriate.
The Hollies supports several care homes
and the post holder will provide medication query support during ward rounds.
The post holder will ensure that the
practice integrates with community and hospital pharmacy to help utilise skill
mix, improve patient outcomes, ensure better access to healthcare and help
manage workload. The role is pivotal to improving the quality of care and
operational efficiencies so requires motivation and passion to deliver
excellent service within general practice.
The post holder will be involved in the
Primary Care Network and oversee Primary Care Network pharmacist work,
providing training and supervision within the clinical team. They will
proactively contribute to practice developments, promoting new ways of working
in their clinical field.
Ideally the post holder will be a
nonmedical prescriber. Although, through mutual agreement between the practice
and the post holder, they may be supported to develop their role to become a
nonmedical prescriber if they have not already acquired the qualification,
dependent on available funding.
