An exciting opportunity to work with One Thornton Heath PCN, a well established primary care network of six GP practices in North Croydon.
The role involves coaching and motivating
patients in managing their own health and well being, to live independently and
improve their health outcomes. The post holder will work with individual and/or
groups of patients and liaise with other healthcare providers. Working for One
Thornton Heath PCN across six GP practices as part of their multidisciplinary
healthcare professional teams, support and supervision will be provided by a GP
Supervisor and/or a line manager. This
is a key position within our developing PCN and appropriate training will be
given.
Main duties of the job
You should have:

Main Duties include:
· Help to improve
the PCN Diabetes & Hypertension challenges and other local health problems
as agreed by the PCN, with appropriate training/support.
· Support group
consultations (virtual as necessary) to support patients to better understand
their condition, how to manage it and agree self-management goals.
· Diabetes
& Hypertension reviews (including bloods/immunisations) for housebound
patients.
· Liaise with
other health care providers as necessary including community diabetes service,
ICN, etc.
About us
One Thornton Heath Primary Care Network is an established network of 6 GP practices with many years' experience of collaborative working.
All the practices are committed to providing high quality care for the 54,000 registered patients within the PCN. This post is part of a team of six 1TH PCN staff currently working successfully across the practices.
The post holder will be employed by a lead practice on behalf of the PCN and the lead pracycie will orovide regular support and clinical supervision, reporting to the PCN Clinical Directors adn Business Lead

Job Summary:
The

their needs, set goals, and support them to implement their personalised health
and care plan.

Provide personalised support to individuals, their families and
carers to ensure that they are active participants in their own healthcare.
· Empower patients to take
more control in managing their own health and wellbeing, to live independently
and improve their health outcomes through interventions such as:
o self-management education
and peer support
o establishing and attaining
goals set by the person based on what is important to them
o working with the social
prescribing service to connect them to community-based activities that support
their health and wellbeing.
· Provide support to local
community groups and work with other health, social care and voluntary sector
providers to support the patients’ health and wellbeing holistically.
· Work with people with
lower activation to understand their level of knowledge, skills and confidence (their
“Activation” level) when engaging with their health and wellbeing.
